Il a été suggéré que le concept de « camouflage » manque de clarté dans la recherche sur l’autisme, et que ses outils de mesure, tels que le Camouflaging Autistic Traits Questionnaire (CAT-Q), pourraient être confondus par d’autres facteurs. Une revue critique de 389 études a été menée afin d’examiner ces problèmes conceptuels et méthodologiques potentiels. Les résultats remettent en question le fait qu’un même construit ait été étudié dans l’ensemble des études, car il existe des incohérences concernant : (a) les termes utilisés ; (b) les références à la littérature conceptuelle établie ; (c) la manière dont les termes sont utilisés (par exemple, de façon interchangeable ou distincte) ; et (d) la manière dont les termes sont définis. Bien que le CAT-Q présente une excellente fiabilité, les données soutenant sa validité sont mitigées, comme le montre sa confusion avec d’autres construits (par exemple, l’anxiété sociale) et sa spécificité limitée à l’autisme. La validité des mesures du camouflage fondées sur les divergences entre informateurs est également remise en question en raison de références insuffisantes à une méthodologie établie. Enfin, la généralisabilité du camouflage à l’ensemble de la population autiste n’est pas claire en raison : (a) de la surreprésentation de femmes autistes diagnostiquées à l’âge adulte ; et (b) de la sous-représentation de personnes autistes présentant des difficultés intellectuelles ou langagières concomitantes, ainsi que de celles ayant des besoins de soutien plus importants. Ces questions sont examinées à la fois sous l’angle de leur pertinence clinique et de la manière dont les recherches futures pourraient les résoudre. Résumé grand public De nombreuses personnes autistes ont déclaré utiliser des stratégies de « camouflage » pour s’adapter au monde social non autiste ou y faire face, et éviter d’être jugées négativement par autrui. Cependant, de nombreux termes ont été utilisés comme synonymes de camouflage, tels que masquage, compensation et gestion des impressions. En raison de cette confusion concernant les termes à utiliser, il a été suggéré que le concept de camouflage manque de clarté et de compréhension, et que cela pourrait contribuer à l’inexactitude des outils utilisés pour mesurer ce comportement. Nous examinons 389 études antérieures afin d’étudier ces préoccupations. Nos résultats confirment ce manque de clarté en montrant que les études sont incohérentes concernant : (a) les termes qu’elles utilisent pour désigner des stratégies comportementales ressemblant au camouflage ; (b) le fait qu’elles se réfèrent ou non à la littérature existante ; (c) le fait qu’elles utilisent des termes différents pour désigner le même concept ou des types de comportement distincts ; et (d) la manière dont elles définissent les termes qu’elles utilisent. Nos résultats remettent également en question l’exactitude des outils de mesure du camouflage, car ces outils pourraient aussi mesurer d’autres comportements (par exemple, l’anxiété sociale) qui ne sont pas vécus uniquement par les personnes autistes. Nous constatons également que les études sur le camouflage se sont principalement concentrées sur des femmes autistes ne présentant pas de difficultés cognitives ou langagières concomitantes et ayant reçu leur diagnostic à l’âge adulte. Bien que le camouflage puisse contribuer au sous-diagnostic de certaines femmes autistes, la plupart des personnes autistes sont des hommes et reçoivent un diagnostic durant la petite enfance, et un grand nombre de personnes autistes présentent effectivement ces autres difficultés. De ce fait, il n’est pas clair que les résultats des études antérieures sur le camouflage puissent s’appliquer à toutes les personnes autistes. Nous formulons quelques suggestions sur la manière dont les chercheurs pourraient résoudre certaines de ces questions dans leurs futures études, et nous examinons ce que ces résultats signifient pour les cliniciens travaillant avec des personnes autistes.
