Contexte : Par rapport aux adultes de la population generale, les adultes autistes sont plus susceptibles de connaitre une mauvaise sante mentale, ce qui peut contribuer a une suicidalite accrue. Bien que la communaute autiste identifie depuis longtemps l'epuisement autistique comme un risque important pour la sante mentale, a ce jour, une seule etude a ete publiee. Les premieres recherches ont mis en evidence l'impact nocif de l'epuisement autistique chez les adultes autistes et le besoin urgent de mieux comprendre ce phenomene. Methodes : Afin de comprendre les experiences vecues des adultes autistes, nous avons utilise l'extraction de donnees pour extraire des publications publiques sur l'epuisement autistique provenant de 2 plateformes en ligne, publiees entre 2005 et 2019, ce qui a permis d'obtenir 1127 publications. En utilisant une analyse thematique reflexive et une approche inductive ascendante, nous avons cherche a comprendre l'etiologie, les symptomes et l'impact de l'epuisement autistique, ainsi que les strategies de prevention et de retablissement. Deux chercheurs autistes ayant une experience auto-declaree d'epuisement autistique ont examine les themes et fourni des eclairages et des retours. Resultats : Nous avons identifie huit themes principaux et trois sous-themes dans l'ensemble des donnees. (1) Manque systemique et generalise de sensibilisation a l'autisme. (1.1) Discrimination et stigmatisation. (2) Une affection chronique ou recurrente. (3) Impact direct sur la sante et le bien-etre. (4) Une vie non vecue. (5) Une benediction deguisee ? (6) La conscience de soi et le controle personnel influencent le risque. (6.1) « Il faut suffisamment de ballons pour gerer le poids des rochers. » (7) Masquage : condamne si vous le faites, condamne si vous ne le faites pas. (8) Demandez aux experts. (8.1) Plus forts ensemble. Le theme general etait qu'un manque generalise de sensibilisation et la stigmatisation concernant l'autisme sous-tendent l'epuisement autistique. Conclusions : Nous avons identifie un ensemble de facteurs distincts mais interdependants qui caracterisent l'epuisement autistique comme une affection recurrente pouvant, directement et indirectement, avoir un impact sur le fonctionnement, la sante mentale, la qualite de vie et le bien-etre des personnes autistes. Les resultats suggerent qu'une sensibilisation et une acceptation accrues de l'autisme pourraient etre essentielles a la prevention et au retablissement apres l'epuisement.
