Résumé

Contexte et objectifs Des niveaux élevés de diagnostics concomitants de santé mentale sont rapportés chez les adultes autistes, mais la recherche sur les interventions psychologiques soutenant le bien-être n'en est qu'à ses débuts. Cette revue systématique à méthodes mixtes a caractérisé toutes les interventions psychologiques disponibles pour les adultes autistes, les mesures utilisées pour évaluer leur efficacité, la mesure dans laquelle les interventions ont été conçues ou adaptées pour les personnes autistes, et les éléments théorisés comme contribuant au bien-être. Méthodes Un protocole a été enregistré auprès de PROSPERO (CRD42023385232) avant le début de l'étude. La revue a utilisé les lignes directrices Preferred Reporting Items for Systematic Reviews and Meta-Analyses. Un processus d'implication des patients et du public a sollicité l'avis d'un groupe d'experts composé de conseillers autistes travaillant avec des personnes autistes afin d'élaborer la stratégie de recherche. Tous les plans d'étude ont été inclus dans la revue afin de répondre à l'objectif de synthétiser les données probantes disponibles depuis 2012, année où le National Institute for Health and Care Excellence a introduit des lignes directrices cliniques pour le travail avec les adultes autistes au Royaume-Uni. Le dépistage a été réalisé sur 100 % des références par deux évaluateurs indépendants. Les risques de biais susceptibles de survenir spécifiquement dans la recherche sur l'autisme ont été évalués. L'évaluation de la qualité a été réalisée à l'aide du Quality Assessment Tool for Studies with Diverse Designs. Des synthèses à méthodes mixtes et qualitatives ont été utilisées pour résumer les résultats. De même, peu d'études ont décrit les mécanismes de changement théorisés du bien-être rapporté ou ont conçu des interventions spécifiquement destinées aux personnes autistes. Conclusions Il existe peu d'études empiriques sur les interventions psychologiques destinées aux adultes autistes, et des définitions imprécises de ce que signifie le bien-être pour les personnes autistes témoignent d'une ambiguïté conceptuelle et méthodologique. Une discussion plus approfondie est nécessaire afin de convenir d'une hiérarchisation communautaire de la pertinence des résultats en matière de santé mentale. Une minorité d'études (n = 29) a utilisé des mesures de résultats conçues pour des personnes autistes ou étalonnées auprès d'elles. Les interventions thérapeutiques les plus courantes reposaient sur la pleine conscience ou les thérapies cognitivo-comportementales, ce qui suggère un choix limité d'interventions psychologiques fondées sur des données probantes pour les personnes autistes et les praticiens. Implications La recherche visant à améliorer le bien-être des adultes autistes en est encore à un stade précoce de développement et se concentre principalement sur l'évaluation de la faisabilité des interventions. Les études d'intervention actuelles reposent sur des mesures de résultats dont l'adéquation pour évaluer des changements significatifs chez les adultes autistes est inconnue. Cette revue fournit un point de référence essentiel pour les chercheurs cliniciens et les cliniciens exerçant en pratique. Une liste des mesures de résultats validées pour une utilisation auprès de personnes autistes est présentée comme résultat. Les résultats s'ajoutent à la littérature préconisant des pratiques cliniques et de recherche affirmant la neurodiversité, qui intègrent la phénoménologie autiste et recommandent une approche critique des interventions psychologiques.

Background and aims There are high levels of co-occurring mental health diagnoses reported for autistic adults, yet research into psychological interventions that support wellbeing is only just emerging. This mixed-methods systematic review characterized all available psychological interventions for autistic adults, the measures used to evaluate their effectiveness, the extent to which the interventions have been designed or adapted for autistic people, and the elements theorized to contribute to wellbeing. Methods A protocol was registered with PROSPERO (CRD42023385232) before commencement. The review used Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. A Patient and Public Involvement process sought input from an expert panel of autistic counselors who work with autistic people to develop the search strategy. All study designs were included in the review to meet the objective of summarizing the available evidence since 2012, the point at which National Institute for Health and Care Excellence introduced Clinical Guidelines for working with autistic adults in the United Kingdom. Screening was performed on 100% of the records by two independent reviewers. Risks of bias that can occur in autism research specifically were assessed. Quality appraisal was undertaken using the Quality Assessment Tool for Studies with Diverse Designs. Mixed-methods and qualitative syntheses were used to summarize the findings. Main contribution A total of 4,186 papers were identified from the literature search, resulting in 69 papers meeting the criteria for inclusion. The majority (n = 42) were pilot or exploratory designs. Few studies identified how adaptations to existing therapeutic interventions affected wellbeing outcomes. Similarly, few studies described the theorized mechanisms of change in reported wellbeing or designed interventions specifically for autistic people. Conclusions There are few empirical studies of psychological interventions for autistic adults, and imprecise definitions of what wellbeing means for autistic people demonstrate conceptual and methodological ambiguity. Further discussion to agree community-based prioritization of the relevance of mental health outcomes is required. A minority of studies (n = 29) used outcome measures designed for, or standardized with, autistic people. The most common therapeutic interventions were based on mindfulness or cognitive-behavioral therapies, suggesting limited choice of evidence-based psychological interventions for autistic people and practitioners. Implications Research into improving the wellbeing of autistic adults is still at an early stage of development, principally focused on assessing the feasibility of interventions. Current intervention studies are reliant on outcome measures with unknown suitability to evaluate meaningful change for autistic adults. This review provides an essential point of reference for clinical researchers and practising clinicians. A list of outcome measures that have been validated for use with autistic people is shared as a finding. The findings add to literature advocating for neurodiversity-affirmative clinical and research practices, which incorporate autistic phenomenology and recommend a critical approach towards psychological interventions.
