Résumé

CONTEXTE : Bien qu'il existe des facteurs de risque connus de suicidalité chez les adultes autistes, ceux-ci sont souvent déconnectés des cadres théoriques qui pourraient expliquer pourquoi le risque est élevé et orienter les interventions cliniques. La présente étude a examiné la pertinence des construits de la théorie interpersonnelle du suicide (ITS), y compris le sentiment d'être une charge, le sentiment d'appartenance contrarié et la capacité acquise pour le suicide, et a exploré les mécanismes par lesquels certains facteurs de risque (statut relationnel, âge au diagnostic) pourraient accroître le risque de suicide. MÉTHODES : Des adultes autistes (n = 314) ont participé à une étude en ligne comprenant des mesures de la dépression, de l'anxiété et des construits de l'ITS. Des analyses de régression linéaire et multinomiale ont dissocié les contributions des variables de l'ITS des effets de la dépression et de l'anxiété sur les idées suicidaires au cours de l'année écoulée, ainsi que sur les tentatives de suicide au cours de l'année écoulée et au cours de la vie. Des analyses de médiation ont examiné les associations entre les facteurs de risque et ces issues suicidaires par l'intermédiaire des mécanismes proposés par l'ITS. RÉSULTATS : Les idées suicidaires au cours de l'année écoulée étaient associées au sentiment d'être une charge, à la répétition mentale de plans suicidaires (une facette de la capacité acquise) et à la dépression. Des sentiments plus importants d'être une charge et une peur réduite de la mort distinguaient les participants ayant fait une tentative de suicide de ceux ayant éprouvé des idées suicidaires au cours de l'année écoulée. Le statut relationnel était indirectement associé aux idées suicidaires au cours de l'année écoulée par l'intermédiaire de la dépression et du sentiment d'être une charge, et était associé aux tentatives au cours de l'année écoulée par son effet sur les idées suicidaires. L'âge au diagnostic n'était lié à aucune variable. LIMITES : La recherche transversale est peu sensible à la causalité et aux dynamiques temporelles, ce qui explique probablement pourquoi les hypothèses d'interaction issues de l'ITS n'ont pas été étayées. Les mesures normatives peuvent être non valides dans les échantillons autistes. Il n'y avait pas de groupe témoin. BACKGROUND: While there are known risk factors for suicidality in autistic adults, these are often unconnected from theoretical frameworks that might explain why risk is elevated and guide clinical interventions. The present study investigated the relevance of constructs from the Interpersonal Theory of Suicide (ITS), including perceived burdensomeness, thwarted belongingness and acquired capability for suicide, and explored mechanisms through which certain risk factors (relationship status, age at diagnosis) might elevate suicide risk. METHODS: Autistic adults (n = 314) completed an online study including measures of depression, anxiety and constructs from the ITS. Linear and multinomial regression analysis disentangled contributions of ITS variables from effects of depression and anxiety for past-year suicide ideation, past-year and lifetime suicide attempts. Mediation analyses examined associations between risk factors and these suicide outcomes via mechanisms proposed by the ITS. RESULTS: Past-year suicide ideation was associated with burdensomeness, mental rehearsal of suicide plans (a facet of acquired capability), and depression. Greater feelings of burdensomeness, and reduced fear of death, marked out participants who had attempted suicide in comparison to those who had experienced suicide ideation in the past year. Relationship status was indirectly associated with past-year suicide ideation via the mediators of depression and burdensomeness, and was associated with past-year attempts via its effect on ideation. Age at diagnosis was unrelated to any variables. LIMITATIONS: Cross-sectional research is insensitive to causality and temporal dynamics, which is likely why interaction hypotheses from the ITS were unsupported. Normative measures may be invalid in autistic samples. There was no control group. The autistic sample was unrepresentative of the whole population, particularly autistic people with intellectual disabilities, ethnic/racial minorities, and gender minorities. CONCLUSIONS: Perceived burdensomeness and acquired capability appear potentially important to suicide in autistic people, and may mediate the effects of some risk factors. Future research should explore the temporal dynamics of suicide trajectories in longitudinal, prospective designs.
