Résumé

Il est souvent rapporté que les personnes présentant un trouble du spectre de l'autisme (TSA) manifestent une indifférence apparente à la douleur ou à la température. Les principaux modèles suggèrent que ce comportement résulte de seuils perceptifs élevés pour les stimuli thermiques, mais les données étayant ces affirmations ne sont pas concluantes. Une proposition alternative suggère que les caractéristiques sensorielles du TSA découlent d'une variabilité perceptive intra-individuelle accrue. Dans cette étude, nous avons mesuré les seuils de détection du chaud et du froid par la méthode des limites chez 142 personnes (83 avec un TSA, 59 avec un développement typique [DT], âgées de 7 à 54 ans), en examinant les relations avec le groupe diagnostique, les caractéristiques démographiques et les mesures cliniques. Nous avons également étudié la relation entre les seuils de détection et une nouvelle mesure de la variabilité intra-individuelle (d'essai à essai) des seuils, un indice présumé de « bruit perceptif ». Cette étude n'a mis en évidence aucune différence dans les seuils de détection thermique entre les personnes avec un TSA et les témoins typiques, malgré de grandes différences entre les groupes dans les questionnaires sur la réactivité sensorielle et de modestes différences de groupe dans la variabilité intra-individuelle. Un QI de performance plus faible, le sexe masculin et une variabilité intra-individuelle plus élevée des estimations de seuil étaient les prédicteurs les plus significatifs de seuils de détection élevés. Bien qu'aucune mesure psychophysique ne soit significativement corrélée aux mesures par questionnaire de l'hyporéactivité sensorielle, une grande variabilité intra-individuelle peut expliquer partiellement les seuils psychophysiques élevés observés dans un sous-groupe de la population TSA.

Individuals with autism spectrum disorder (ASD) are often reported to exhibit an apparent indifference to pain or temperature. Leading models suggest that this behavior is the result of elevated perceptual thresholds for thermal stimuli, but data to support these assertions are inconclusive. An alternative proposal suggests that the sensory features of ASD arise from increased intra-individual perceptual variability. In this study, we measured method-of-limits warm and cool detection thresholds in 142 individuals (83 with ASD, 59 with typical development [TD], aged 7-54 years), testing relationships with diagnostic group, demographics, and clinical measures. We also investigated the relationship between detection thresholds and a novel measure of intra-individual (trial-to-trial) threshold variability, a putative index of "perceptual noise." This investigation found no differences in thermal detection thresholds between individuals with ASD and typical controls, despite large differences between groups in sensory reactivity questionnaires and modest group differences in intra-individual variability. Lower performance IQ, male sex, and higher intra-individual variability in threshold estimates were the most significant predictors of elevated detection thresholds. Although no psychophysical measure was significantly correlated with questionnaire measures of sensory hyporeactivity, large intra-individual variability may partially explain the elevated psychophysical thresholds seen in a subset of the ASD population.
