Résumé

Résumé Contexte Des différences dans la réponse aux stimuli sensoriels, notamment l'hyperréactivité sensorielle (HYPER), l'hyporéactivité (HYPO) et la recherche sensorielle (SEEK), ont été observées chez les personnes autistes à travers les modalités sensorielles, mais peu d'études ont examiné la structure de ces traits « supramodaux » dans la population autiste. Méthodes S'appuyant sur un échantillon combiné de 3868 jeunes autistes issus de 12 sources de données distinctes (âgés de 3 à 18 ans et représentant l'ensemble des niveaux de capacités cognitives), la présente étude a utilisé des techniques psychométriques et méta-analytiques modernes pour examiner la structure latente et les corrélats de HYPER, HYPO et SEEK rapportés par les aidants, au sein des modalités sensorielles et entre elles. Des indices statistiques bifactoriels ont été utilisés à la fois pour évaluer la force d'un facteur de « schéma général de réponse » pour chaque construit supramodal et pour déterminer la valeur ajoutée des scores de « schéma de réponse spécifique à la modalité » (par exemple, HYPER visuel). Des modèles bayésiens d'analyse intégrative de données à effets aléatoires ont été utilisés pour examiner les corrélats cliniques et démographiques de tous les (sous-)construits HYPER, HYPO et SEEK interprétables. Résultats Tous les sous-construits HYPER spécifiques à une modalité pouvaient être mesurés de manière fiable et valide, tandis que certains sous-construits HYPO et SEEK spécifiques à une modalité étaient psychométriquement inadéquats lorsqu'ils étaient mesurés à l'aide des items existants. Les analyses bifactorielle ont soutenu la validité d'un construit HYPER supramodal (ω H =.800), mais pas d'un construit HYPO supramodal (ω H =.653), et les modèles SEEK supramodaux ont suggéré une version plus limitée du construit qui excluait certaines modalités sensorielles (ω H =.800 ; 4/7 modalités). Les sous-échelles spécifiques à une modalité ont démontré une valeur ajoutée significative pour tous les schémas de réponse. Les corrélations méta-analytiques variaient selon le construit, bien que les caractéristiques sensorielles aient eu tendance à être les plus corrélées avec d'autres domaines des caractéristiques centrales de l'autisme et des symptômes psychiatriques concomitants (HYPER général et HYPO de la parole présentant les plus grands nombres de corrélations significatives sur le plan pratique). Limites Les conclusions pourraient ne pas être généralisables au-delà du pool spécifique d'items utilisé dans la présente étude, qui se limitait aux rapports des aidants sur des comportements observables et excluait les items multisensoriels reflétant de nombreuses expériences sensorielles « du monde réel ». Conclusion Parmi les trois schémas de réponse sensorielle, seul HYPER a démontré des preuves suffisantes pour une interprétation valide au niveau supramodal, tandis que les construits HYPO/SEEK supramodaux ont démontré des limitations psychométriques substantielles. Pour les cliniciens et les chercheurs cherchant à caractériser la réactivité sensorielle dans l'autisme, les scores de schéma de réponse spécifiques à une modalité peuvent représenter des alternatives viables qui surmontent nombre de ces limitations.

Abstract Background Differences in responding to sensory stimuli, including sensory hyperreactivity (HYPER), hyporeactivity (HYPO), and sensory seeking (SEEK) have been observed in autistic individuals across sensory modalities, but few studies have examined the structure of these “supra-modal” traits in the autistic population. Methods Leveraging a combined sample of 3868 autistic youth drawn from 12 distinct data sources (ages 3–18 years and representing the full range of cognitive ability), the current study used modern psychometric and meta-analytic techniques to interrogate the latent structure and correlates of caregiver-reported HYPER, HYPO, and SEEK within and across sensory modalities. Bifactor statistical indices were used to both evaluate the strength of a “general response pattern” factor for each supra-modal construct and determine the added value of “modality-specific response pattern” scores (e.g., Visual HYPER). Bayesian random-effects integrative data analysis models were used to examine the clinical and demographic correlates of all interpretable HYPER, HYPO, and SEEK (sub)constructs. Results All modality-specific HYPER subconstructs could be reliably and validly measured, whereas certain modality-specific HYPO and SEEK subconstructs were psychometrically inadequate when measured using existing items. Bifactor analyses supported the validity of a supra-modal HYPER construct (ω H =.800) but not a supra-modal HYPO construct (ω H =.653), and supra-modal SEEK models suggested a more limited version of the construct that excluded some sensory modalities (ω H =.800; 4/7 modalities). Modality-specific subscales demonstrated significant added value for all response patterns. Meta-analytic correlations varied by construct, although sensory features tended to correlate most with other domains of core autism features and co-occurring psychiatric symptoms (with general HYPER and speech HYPO demonstrating the largest numbers of practically significant correlations). Limitations Conclusions may not be generalizable beyond the specific pool of items used in the current study, which was limited to caregiver report of observable behaviors and excluded multisensory items that reflect many “real-world” sensory experiences. Conclusion Of the three sensory response patterns, only HYPER demonstrated sufficient evidence for valid interpretation at the supra-modal level, whereas supra-modal HYPO/SEEK constructs demonstrated substantial psychometric limitations. For clinicians and researchers seeking to characterize sensory reactivity in autism, modality-specific response pattern scores may represent viable alternatives that overcome many of these limitations.
