CONTEXTE : Les personnes autistes présentent des profils atypiques de traitement sensoriel dont il est connu qu’ils sont liés à la qualité de vie, mais qui sont également très hétérogènes. Les investigations antérieures de cette hétérogénéité ont habituellement utilisé des questionnaires et ont rarement étudié le traitement sensoriel dans le développement typique (TD) parallèlement au développement du spectre de l’autisme (ASD). MÉTHODES : La présente étude a utilisé une classification hiérarchique dans un large échantillon afin d’identifier des sous-groupes de jeunes enfants autistes et au développement typique sur la base de la puissance globale de champ (GFP) normalisée de leurs potentiels évoqués liés aux événements (ERP) en réponse à des stimuli auditifs de quatre intensités sonores différentes (50, 60, 70, 80 dB SPL) : c’est-à-dire sur la base d’un indice des forces relatives de leurs réponses neuronales dans ces conditions d’intensité sonore. RÉSULTATS : Quatre groupes de participants ont été définis. Les réponses GFP normalisées à des sons d’intensités différentes différaient fortement entre les groupes. Il existait un chevauchement considérable dans l’attribution aux groupes des participants autistes et au développement typique, mais les participants autistes étaient plus susceptibles de présenter un profil d’augmentations relativement linéaires de la force de réponse, accompagné d’une réponse disproportionnellement forte aux stimuli de 70 dB. Les participants autistes présentant ce profil tendaient à obtenir des scores plus élevés aux évaluations des capacités cognitives. Une tendance a également été observée selon laquelle les participants au développement typique appartenaient de manière disproportionnée à un groupe caractérisé par des réponses de 60 dB disproportionnellement et non linéairement fortes. Une plus grande distractibilité auditive a été rapportée chez les participants autistes d’un groupe caractérisé par des réponses disproportionnellement fortes aux sons les plus forts (80 dB) et, en outre, des réponses relativement fortes aux sons forts étaient corrélées à la distractibilité auditive. Cela semble fournir des preuves de l’existence concomitante d’atypies sensorielles comportementales et neuronales. LIMITES : Une réplication pourrait être nécessaire pour vérifier les résultats exploratoires. Cette analyse n’aborde pas la variabilité liée aux latences et aux topographies classiques des ERP. Le questionnaire sensoriel utilisé n’a pas été spécifiquement conçu pour une utilisation dans l’autisme. L’acuité auditive n’a pas été mesurée. La variabilité des réponses sensorielles non liée à l’intensité sonore n’est pas abordée, ce qui laisse place à des recherches supplémentaires. CONCLUSIONS : Dans l’ensemble, ces données démontrent les avantages plus larges de l’utilisation de l’électrophysiologie pour explorer les différences individuelles. Elles mettent en lumière différents profils de réponses neuronales et suggèrent des relations entre les réponses neuronales sensorielles et les comportements sensoriels, les capacités cognitives et le statut diagnostique de l’autisme.
