La mauvaise santé mentale, comparativement à celle de la population d'enfants neurotypiques, constitue une préoccupation grave pour de nombreux enfants et jeunes autistes dans le monde. Au Royaume-Uni, le nombre de jeunes autistes recevant des soins dans des établissements de santé mentale en hospitalisation financés par le NHS augmente. Alors que les différences de traitement sensoriel ont désormais été ajoutées aux critères diagnostiques internationaux de l'autisme, de récentes recherches fondées sur la pratique, menées par des autistes et coproduites, commandées par le Children and Young People’s Mental Health Taskforce et réalisées par le National Development Team for Inclusion, ont mis en évidence que les connaissances relatives aux différences et aux besoins sensoriels des personnes autistes sont institutionnellement absentes. En particulier, il a été constaté que les environnements sensoriels des établissements d'hospitalisation financés par NHS England présentent parfois des difficultés extrêmes pour les jeunes autistes qui, au mieux, entravent le bien-être et, au pire, exacerbent les problèmes de santé mentale existants, déclenchant pour certains un cycle de progression vers des cadres de plus en plus restrictifs. Cet article présente certains de ces enseignements, tirés de la consultation de jeunes personnes autistes ayant l'expérience des services d'hospitalisation et d'Experts by Experience autistes travaillant sur de nouvelles évaluations des environnements sensoriels des unités. Nous présentons d'abord la conception de l'autisme comme étant principalement façonné par des différences de traitement sensoriel et social, et exposons l'importance de cette perspective pour les soins hospitaliers des jeunes et des enfants autistes. Nous donnons ensuite un aperçu des difficultés sensorielles actuelles qui existent dans les établissements de santé mentale en hospitalisation pour les enfants et les jeunes autistes. Enfin, nous concluons par quelques suggestions de domaines de recherche futurs concernant l'impact de l'adaptation des environnements des unités, qui sont prometteurs pour des contextes plus larges et internationaux.
