Contexte : Les personnes autistes sont confrontées à de multiples obstacles aux soins de santé. Afin de reconnaître des obstacles spécifiques de manière standardisée, les personnes autistes, les prestataires de soins de santé et les chercheurs ont besoin d’un outil de mesure approprié. L’Academic Autistic Spectrum Partnership in Research and Education a développé avec succès la liste de contrôle des obstacles aux soins de santé — forme courte (BHC-SF). Il reste à déterminer si (1) une version allemande de la BHC-SF présente de bonnes propriétés psychométriques et si (2) la BHC-SF se compose de plusieurs construits (c’est-à-dire en évaluant la validité structurelle). Méthodes : Nous avons créé une version traduite et culturellement adaptée de la BHC-SF à 17 items et l’avons utilisée auprès de 345 personnes autistes en Allemagne. Les analyses psychométriques comprenaient des calculs visant à tester la fiabilité de la cohérence interne (alpha de Cronbach) et la validité de construit (c’est-à-dire des tests d’hypothèses a priori concernant les corrélations avec l’arrêt antérieur de traitements et des déclarations sur les obstacles aux soins de santé en termes de manque de connaissances, de participation, de besoins spécifiques à l’autisme, de manque de services, d’accès/de financement et de collaboration). Nous avons réalisé une analyse factorielle exploratoire (EFA) afin d’étudier la présence de sous-échelles de la BHC-SF (c’est-à-dire la validité structurelle) et avons effectué des calculs de fiabilité et de validité lorsque cela était approprié. Résultats : Le score total moyen de la BHC-SF était de moyenne = 8,05 (écart-type = 4,05), avec davantage d’obstacles chez les personnes s’identifiant comme femmes par rapport aux hommes. Le score total de la BHC-SF a révélé une bonne fiabilité de cohérence interne (alpha de Cronbach = 0,82). L’évaluation de la validité de construit a principalement soutenu les associations hypothétisées. L’EFA a identifié quatre sous-échelles de la BHC-SF, pour lesquelles nous avons trouvé un soutien psychométrique plus faible en ce qui concerne la fiabilité de cohérence interne et la validité de construit. Conclusions : La version allemande de la BHC-SF démontre une bonne fiabilité de cohérence interne et une bonne validité de construit. Nous avons identifié quatre construits différents sous-jacents à la BHC-SF. Les sous-échelles dérivées nécessitent des tests psychométriques supplémentaires. L’utilisation du score total de la BHC-SF pourrait potentiellement aider les prestataires de soins de santé et les chercheurs à identifier et à traiter systématiquement les obstacles aux soins de santé pour les patients autistes.
