Résumé

Objectif – Les adultes sur le spectre de l’autisme rencontrent des difficultés pour recevoir des soins de santé, et les prestataires de soins de santé rencontrent des difficultés pour proposer des soins de santé aux adultes sur le spectre de l’autisme. L’objectif de cet article est, premièrement, d’évaluer les diverses difficultés et, deuxièmement, de fournir des stratégies pour les surmonter. Conception/méthodologie/approche – Dans ce projet de recherche qualitative, les obstacles et les facilitateurs actuels de l’accès aux services de santé ont été recueillis dans le cadre d’une collaboration entre des personnes autistes engagées dans l’auto-représentation et des professionnels de l’autisme à Berlin, en Allemagne. Les résultats ont été complétés par un examen de lignes directrices pratiques et de recherches sur l’accessibilité des services pour les patients sur le spectre de l’autisme. Résultats – Une liste complète des obstacles aux soins de santé a été établie et structurée selon divers aspects, tels que « prise de rendez-vous », « salle d’attente », « communication » et « examen ». Des stratégies tenant compte des particularités perceptives et communicationnelles de l’autisme se sont révélées améliorer l’accès aux soins de santé pour les adultes autistes. Implications pratiques – Faciliter l’accès au système de soins de santé peut améliorer le diagnostic et le traitement des maladies mentales et somatiques et, par conséquent, l’état de santé et la qualité de vie des personnes sur le spectre de l’autisme. Cette reconnaissance des besoins des adultes sur le spectre de l’autisme peut servir de modèle pour d’autres domaines de la société, tels que l’éducation et l’emploi. Originalité/valeur – L’acquisition de données dans ce projet présente une valeur particulière parce qu’elle a résulté d’une collaboration entre une organisation d’auto-représentation autiste et des professionnels travaillant dans le domaine des déficiences intellectuelles et développementales, prenant en compte les expériences d’adultes autistes sur l’ensemble de la gamme du fonctionnement intellectuel.

Purpose – Adults on the autism spectrum experience difficulties in receiving health care, and health care providers face difficulties in offering health care to adults on the autism spectrum. The purpose of this paper is first, to assess the various difficulties and second, to provide strategies to overcome them. Design/methodology/approach – In this qualitative research project, current barriers and facilitators to health care services were sampled from a collaboration of autistic self-advocates and autism professionals in Berlin, Germany. The findings were complemented by a review of practical guidelines and research about the service accessibility of patients on the autism spectrum. Findings – A comprehensive list of barriers to health care was compiled and structured according to various aspects, such as “making appointments”, “waiting area”, “communication”, and “examination”. Strategies considering the perceptual and communicative peculiarities of autism were found to improve access to health care for autistic adults. Practical implications – Providing access to the health care system may improve the diagnosis and treatment of mental and somatic illnesses, and thereby, the health status and quality of life for people on the autism spectrum. This recognition of the needs of adults on the autism spectrum may serve as a model for other areas in society, such as education and employment. Originality/value – Data acquisition in this project is of special value because it resulted from collaboration between an autistic self-advocacy organization and professionals working in the field of intellectual developmental disabilities considering the experiences of autistic adults in the entire range of intellectual functioning.
