Résumé

OBJECTIF : Les adultes ayant reçu un diagnostic de trouble de la personnalité présentent une forte prévalence d'autisme et de trouble déficitaire de l'attention avec hyperactivité (TDAH) concomitants. Cependant, il n'existe souvent pas de parcours défini au sein des services de santé mentale pour répondre aux besoins de cette population, et les données probantes ou le consensus sur les approches optimales d'identification, de traitement et de soutien des personnes neurodivergentes ayant reçu un diagnostic de trouble de la personnalité font défaut. Les points de vue de cette population sur les soins qu'elle reçoit dans les services communautaires de santé mentale ont été peu explorés. Par conséquent, l'objectif de ce projet était de comprendre les expériences et les perspectives de personnes neurodivergentes ayant reçu un diagnostic de trouble de la personnalité, afin d'éclairer un parcours clinique efficace, sûr, durable et équitable. MÉTHODES : Dix entretiens qualitatifs ont été menés auprès de femmes ayant reçu un diagnostic d'autisme ou de TDAH, ou ayant été orientées pour un tel diagnostic, et ayant reçu un diagnostic de trouble de la personnalité. Les entretiens ont été analysés au moyen d'une analyse thématique réflexive. RÉSULTATS : Cinq thèmes principaux ont émergé : les facteurs liés au personnel (compréhension et compétences, attitudes et communication), les parcours et processus (accès aux services et obstacles au soutien), l'implication et l'autonomisation (au moyen d'adaptations et de l'autonomisation), le soutien et les interventions cliniques (expérience des thérapies individuelles et des groupes, et possibilités d'évaluer le soutien), ainsi que le diagnostic et l'identification (l'impact et l'exactitude du diagnostic). CONCLUSIONS : Cette étude met en évidence des lacunes dans les pratiques actuelles ainsi que des préférences personnelles concernant l'identité et les expériences de diagnostic erroné. Elle identifie les composantes d'un parcours clinique intégré, comprenant une approche de l'évaluation centrée sur la personne et guidée par la formulation, ainsi que des aménagements raisonnables ; un soutien psychosocial dirigé par les pairs ; des thérapies psychologiques transdiagnostiques adaptées ; et une coproduction intégrée. Les priorités cliniques et de recherche sont examinées.

PURPOSE: Adults with a personality disorder diagnosis have a high prevalence of co-occurring autism and attention deficit hyperactivity disorder (ADHD). However, there is often no defined pathway within mental health services to meet the needs of this population, and a lack of evidence or consensus on the optimal approaches to identification, treatment and support for neurodivergent people with a personality disorder diagnosis. There has been little exploration of the views of this population on the care they receive in community mental health services. Therefore, the aim of this project was to understand the experiences and perspectives of neurodivergent people with a personality disorder diagnosis, to inform a clinical pathway which is effective, safe, sustainable and equitable. METHODS: Ten qualitative interviews were conducted with women with a diagnosis of, or had been referred for a diagnosis of, autism or ADHD, and a diagnosis of personality disorder. Interviews were analysed using reflexive thematic analysis. RESULTS: Five key themes emerged; staff factors (understanding and skills, attitudes and communication), pathways and processes (access to services and barriers to support), involving and enabling (through adaptations and empowerment), support and clinical interventions (experience of individual therapies and groups, and opportunities to evaluate support), and diagnosis and identification (the impact and accuracy of diagnosis). CONCLUSIONS: This study highlights gaps in current practice as well as personal preferences about identity and experiences of misdiagnosis. It identifies the components of an integrated clinical pathway, that include a person-centered, formulation-driven approach to assessment and reasonable adjustments; peer-led psychosocial support; adapted transdiagnostic psychological therapies; and embedded co-production. Clinical and research priorities are discussed.
