Résumé

Résumé Contexte Les personnes autistes tendent à avoir une qualité de vie réduite tout au long de la vie. Cette réduction de la qualité de vie résulte des traits autistiques, de la détresse psychique et d'une mauvaise adéquation entre la personne et l'environnement. Dans la présente étude, nous avons examiné le rôle médiateur des problèmes internalisés et externalisés dans la relation entre le fait d'avoir un diagnostic d'autisme et la qualité de vie perçue, selon un plan longitudinal. Méthodes Au total, 66 participants ont été inclus dans un groupe combiné composé d'adultes émergents diagnostiqués avec un trouble du spectre de l'autisme (âge moyen : 22,2 ans) et d'un groupe sans autisme (âge moyen : 20,9 ans), suivis prospectivement pendant dix ans au cours de trois vagues d'évaluation à 12, 14 et 22 ans. Les participants ont rempli le Questionnaire sur la qualité de vie perçue. Les effets totaux et indirects ont été testés dans une analyse de médiation sérielle. Résultats Les résultats ont révélé un effet significatif de l'autisme sur la qualité de vie. Les problèmes internalisés au suivi à deux ans ont significativement médiatisé la qualité de vie à l'âge adulte émergent, contrairement aux problèmes externalisés. L'effet direct de l'autisme sur la qualité de vie n'a pas atteint le seuil de significativité statistique après l'introduction des problèmes internalisés comme médiateur. Conclusion Il existe une corrélation significative entre le fait d'avoir un autisme et une mauvaise qualité de vie ; toutefois, cette corrélation est médiatisée par l'impact des problèmes internalisés à l'adolescence sur la qualité de vie actuellement perçue.

Abstract Background Individuals with autism tend to have a reduced quality of life across the lifespan. This reduced quality of life is a result of autism traits, mental distress, and poor person/environment fit. In the present study, we examined the mediating role of internalizing and externalizing problems on the relationship between having an autism diagnosis and perceived quality of life in a longitudinal design. Methods There were in total 66 participants in a combined group of emerging adults diagnosed with autism spectrum disorder (mean age 22.2 years), and a group without autism (mean age 20.9 years) prospectively followed for ten years through three assessment waves at 12, 14 and 22 years. Participants filled out the Perceived Quality of Life Questionnaire. Total and indirect effects were tested in serial mediation analysis. Results Results revealed a significant effect of autism on quality of life. Internalizing problems at two-year follow-up significantly mediated quality of life in emerging adulthood while externalizing problems did not. The direct effect of autism on quality of life did not reach statistical significance after entering internalizing problems as a mediator. Conclusion There is a significant correlation between having autism and poor quality of life; however, this correlation is mediated by adolescent internalizing problems' impact on the current perceived quality of life.
