Résumé

La qualité de vie est largement reconnue comme l’un des résultats les plus importants de la recherche sur l’autisme, mais peu de mesures de ce construit ont été validées pour une utilisation chez les personnes autistes. L’objectif de la présente étude était d’examiner les propriétés psychométriques du Patient-Reported Outcomes Measurement Information System Global–10, une mesure établie par autoévaluation de la qualité de vie liée à la santé, dans un échantillon de 901 adultes autistes (principalement des femmes et diagnostiqués à l’âge adulte) recrutés au sein de la cohorte Simons Foundation Powering Autism Research for Knowledge. À l’aide d’un cadre fondé sur la théorie de réponse à l’item, nous avons réalisé une évaluation psychométrique complète du Patient-Reported Outcomes Measurement Information System Global–10 dans cet échantillon, en examinant sa structure latente, le fonctionnement différentiel des items, la fiabilité et la validité de construit. Après l’élaboration d’un modèle de mesure spécifique à l’autisme, le Patient-Reported Outcomes Measurement Information System Global–10 a démontré d’excellentes propriétés psychométriques dans l’échantillon actuel, notamment un excellent ajustement entre le modèle et les données, une fiabilité élevée, un fonctionnement différentiel minimal des items entre les sous-groupes d’adultes autistes, et un profil attendu de corrélations avec des variables externes. Des analyses exploratoires ont indiqué qu’une qualité de vie plus faible était associée au sexe féminin et au fait de s’identifier comme membre d’une minorité sexuelle/de genre. Un calculateur de scores en ligne gratuit a été créé afin de faciliter l’utilisation et l’interprétation des scores normés du trait latent de qualité de vie générale du Patient-Reported Outcomes Measurement Information System Global–10 pour des applications cliniques et de recherche (disponible à l’adresse https://asdmeasures.shinyapps.io/promis_qol ). Résumé vulgarisé La qualité de vie est un résultat que les chercheurs et les défenseurs des personnes autistes s’accordent à considérer comme extrêmement important à prendre en compte lors de la mise en œuvre de services, d’interventions et de soutiens destinés aux personnes autistes. Toutefois, peu de recherches ont porté sur la manière dont la qualité de vie peut être mesurée au mieux chez les personnes autistes ou sur la question de savoir si les questionnaires existants sur la qualité de vie sont appropriés pour une utilisation dans la population autiste. Cette étude visait à valider une mesure établie de la qualité de vie, le Patient-Reported Outcomes Measurement Information System Global–10, dans un vaste échantillon d’adultes autistes recrutés en ligne. Nous avons créé une nouvelle méthode de cotation de l’échelle Patient-Reported Outcomes Measurement Information System Global–10 et de génération d’un score de « qualité de vie générale » spécifique aux adultes autistes. Ce nouveau score s’est très bien comporté dans cet échantillon, présentant très peu d’erreur de mesure et étant lié de manière attendue à des construits similaires, tels que la santé physique et la détresse émotionnelle. Des analyses exploratoires ont montré qu’une qualité de vie plus faible était associée au sexe féminin et à l’auto-identification comme membre d’une minorité sexuelle ou de genre (c’est-à-dire une identité LGBTQ +). Ces résultats suggèrent que le nouveau score de qualité de vie du Patient-Reported Outcomes Measurement Information System Global–10 est une mesure fiable et valide de la qualité de vie chez les adultes autistes, bien que des études supplémentaires soient nécessaires pour explorer davantage ses propriétés de mesure dans d’autres sous-groupes de la population autiste, tels que les personnes présentant une déficience intellectuelle. Cette mesure est disponible gratuitement pour être utilisée comme résultat tant dans la recherche que dans la pratique clinique, et un calculateur de scores en ligne est disponible pour soutenir l’utilisation de cette mesure dans des applications réelles.

Quality of life is widely acknowledged as one of the most important outcomes in autism research, but few measures of this construct have been validated for use in autistic people. The goal of the current study was to examine the psychometric properties of the Patient-Reported Outcomes Measurement Information System Global–10, an established self-report measure of health-related quality of life, in a sample of 901 autistic adults (predominantly female and adult-diagnosed) recruited from the Simons Foundation Powering Autism Research for Knowledge cohort. Using an item response theory framework, we performed a comprehensive psychometric evaluation of the Patient-Reported Outcomes Measurement Information System Global–10 in this sample, examining its latent structure, differential item functioning, reliability, and construct validity. After developing an autism-specific measurement model, the Patient-Reported Outcomes Measurement Information System Global–10 demonstrated excellent psychometric properties in the current sample, including excellent model-data fit, high reliability, minimal differential item functioning across subgroups of autistic adults, and an expected pattern of correlations with external variables. Exploratory analyses indicated that lower quality of life was associated with female sex and identified as a sexual/gender minority. A free online score calculator has been created to facilitate the use and interpretation of normed Patient-Reported Outcomes Measurement Information System Global–10 general quality of life latent trait scores for clinical and research applications (available at https://asdmeasures.shinyapps.io/promis_qol ). Lay Abstract Quality of Life an outcome that both researchers and autistic advocates agree is extremely important to consider when implementing services, interventions, and supports for autistic people. However, there has been little research on the topic of how quality of life can best be measured in autistic people or whether existing quality of life questionnaires are appropriate for use in the autistic population. This study aimed to validate an established quality of life measure, the Patient-Reported Outcomes Measurement Information System Global–10, in a large sample of autistic adults recruited online. We created a new way to score the Patient-Reported Outcomes Measurement Information System Global–10 scale and generate a “General quality of life” score specific to autistic adults. This new score performed very well in this sample, showing very little measurement error and relating in expected ways to similar constructs, such as physical health and emotional distress. Exploratory analyses found that lower quality of life was associated with female sex and self-identification as a sexual or gender minority (i.e. LGBTQ + identity). These findings suggest that the new Patient-Reported Outcomes Measurement Information System Global–10 quality of life score is a reliable and valid measure of quality of life in autistic adults, although additional studies are necessary to further explore its measurement properties in other subsets of the autistic population, such as individuals with intellectual disabilities. This measure is freely available for use as an outcome in both research and clinical practice, and an online score calculator is available to support the use of this measure in real-world applications.
