Résumé

Résumé Contexte Les adultes autistes, en particulier les femmes, sont plus susceptibles de connaître une mauvaise santé chronique que la population générale. Les syndromes de sensibilité centrale (CSS) constituent un groupe d'affections apparentées dont on pense qu'elles impliquent une sensibilisation sous-jacente du système nerveux central ; une sensibilité sensorielle accrue est une caractéristique fréquente. Des données anecdotiques suggèrent que les adultes autistes pourraient être plus susceptibles de développer un CSS. Cette étude visait à examiner la survenue de diagnostics et de symptômes de CSS chez des adultes autistes, et à déterminer si les symptômes de CSS étaient liés aux traits autistiques, à la santé mentale, à la sensibilité sensorielle ou au genre. Méthodes L'échantillon complet de participants comprenait 973 adultes autistes (410 hommes, 563 femmes, âge moyen = 44,6) inscrits au Registre néerlandais de l'autisme, qui ont rempli des questionnaires évaluant les traits autistiques, la sensibilité sensorielle, les CSS, ainsi que les symptômes de santé physique et mentale. La fiabilité et la validité du Central Sensitization Inventory (CSI) dans un échantillon autiste ont été établies à l'aide d'analyses factorielles exploratoires et confirmatoires. Des analyses du chi carré, des tests t indépendants, une régression hiérarchique et une analyse de cheminement ont été utilisés pour analyser les relations entre les symptômes de CSS, les traits autistiques, les mesures de santé mentale et de bien-être, la sensibilité sensorielle, l'âge et le genre. Résultats 21 % des participants ont signalé un ou plusieurs diagnostics de CSS, et 60 % ont obtenu un score égal ou supérieur au seuil clinique pour un CSS. Les femmes autistes étaient plus susceptibles de signaler un diagnostic de CSS et présentaient davantage de symptômes de CSS que les hommes. La sensibilité sensorielle, l'anxiété, l'âge et le genre étaient des prédicteurs significatifs des symptômes de CSS, la sensibilité sensorielle et l'anxiété médiatisant entièrement la relation entre les traits autistiques et les symptômes de CSS. Limites Bien que cette étude ait inclus un large échantillon d'adultes autistes, nous ne disposions pas d'un groupe témoin ni d'un groupe CSS uniquement. Nous ne pouvions pas non plus inclure un groupe non binaire en raison d'un manque de puissance statistique. Conclusions Les diagnostics et symptômes de CSS semblent très fréquents dans la population autiste. Une sensibilisation accrue à une association entre l'autisme et la sensibilisation centrale devrait éclairer les cliniciens et orienter la pratique diagnostique, en particulier pour les femmes chez lesquelles les CSS sont fréquents et l'autisme sous-reconnu.

Abstract Background Autistic adults, particularly women, are more likely to experience chronic ill health than the general population. Central sensitivity syndromes (CSS) are a group of related conditions that are thought to include an underlying sensitisation of the central nervous system; heightened sensory sensitivity is a common feature. Anecdotal evidence suggests autistic adults may be more prone to developing a CSS. This study aimed to investigate the occurrence of CSS diagnoses and symptoms in autistic adults, and to explore whether CSS symptoms were related to autistic traits, mental health, sensory sensitivity, or gender. Methods The full sample of participants included 973 autistic adults (410 men, 563 women, mean age = 44.6) registered at the Netherlands Autism Register, who completed questionnaires assessing autistic traits, sensory sensitivity, CSS, physical and mental health symptoms. The reliability and validity of the Central Sensitization Inventory (CSI) in an autistic sample was established using exploratory and confirmatory factor analyses. Chi 2 analyses, independent t -tests, hierarchical regression and path analysis were used to analyse relationships between CSS symptoms, autistic traits, measures of mental health and wellbeing, sensory sensitivity, age and gender. Results 21% of participants reported one or more CSS diagnosis, and 60% scored at or above the clinical cut-off for a CSS. Autistic women were more likely to report a CSS diagnosis and experienced more CSS symptoms than men. Sensory sensitivity, anxiety, age and gender were significant predictors of CSS symptoms, with sensory sensitivity and anxiety fully mediating the relationship between autistic traits and CSS symptoms. Limitations Although this study included a large sample of autistic adults, we did not have a control group or a CSS only group. We also could not include a non-binary group due to lack of statistical power. Conclusions CSS diagnoses and symptoms appear to be very common in the autistic population. Increased awareness of an association between autism and central sensitisation should inform clinicians and guide diagnostic practice, particularly for women where CSS are common and autism under recognised.
