Résumé Des recherches antérieures ont indiqué que les personnes autistes rapportent une qualité de vie (QdV) plus faible que les personnes non autistes. Il n’est pas clair si ce sont les traits autistiques eux-mêmes ou des styles de pensée concomitants ou des difficultés de santé mentale qui altèrent le plus la QdV. Cette étude a testé un modèle hypothétique afin d’explorer comment l’« intolérance à l’incertitude » (II), l’alexithymie (difficulté à identifier et à décrire ses propres émotions) et l’anxiété interviennent dans l’association entre les traits autistiques et la QdV. Des données issues d’une enquête en ligne ont été analysées auprès de 116 adultes autistes et de 51 adultes non autistes ayant rempli six questionnaires standardisés mesurant les traits autistiques, l’alexithymie, l’II, l’anxiété et la QdV (domaines de la santé physique, de la santé psychologique, des relations sociales et de l’environnement). Par rapport au groupe non autiste, le groupe autiste a rapporté des scores plus élevés d’alexithymie, d’II et d’anxiété, ainsi que des scores de QdV plus faibles dans tous les domaines. Dans l’ensemble de l’échantillon, les traits autistiques, l’alexithymie, l’II et l’anxiété étaient positivement corrélés les uns aux autres et négativement associés aux quatre domaines de la QdV. Enfin, l’II et l’anxiété ont partiellement médiatisé en série les voies allant des traits autistiques aux domaines de la santé physique et de l’environnement de la QdV, et ont entièrement médiatisé les voies allant des traits autistiques aux domaines de la santé psychologique et des relations sociales de la QdV, dans l’ensemble de l’échantillon. La QdV plus faible vécue par les personnes autistes peut s’expliquer en partie par l’effet médiateur à la fois de l’II et de l’anxiété (mais non de l’alexithymie). Cette étude souligne la nécessité d’interventions fondées sur des données probantes pour traiter à la fois l’II et l’anxiété afin d’améliorer la QdV des personnes autistes et de celles présentant des niveaux élevés de traits autistiques.

Abstract Previous research has indicated that autistic individuals report lower quality of life (QoL) than non-autistic people. It is unclear whether it is the autism traits themselves or co-occurring thinking styles or mental health difficulties that most impair QoL. This study tested a hypothesised model to explore how ‘intolerance of uncertainty’ (IU), alexithymia (difficulty in identifying and describing own emotions), and anxiety play into the association between autistic traits and QoL. Online survey data were analysed from 116 autistic and 51 non-autistic adults who completed six standardised questionnaires measuring autistic traits, alexithymia, IU, anxiety and QoL (physical health, psychological health, social relations, and environment domains). The autistic group reported higher scores for alexithymia, IU and anxiety, and lower scores for QoL across domains, compared to the non-autistic group. Across the entire sample, autistic traits, alexithymia, IU and anxiety were positively correlated with one another, and negatively related to the four domains of QoL. Finally, IU and anxiety partially serially mediated the pathways from autistic traits to physical health and environment domains of QoL, and fully mediated the pathways from autistic traits to psychological health and social relations domains of QoL, across the full sample. The lower QoL experienced by autistic people may be explained in part by the mediating effect of both IU and anxiety (but not alexithymia). This study highlights the need for evidence-based interventions to address both IU and anxiety to improve QoL for autistic people/those with high levels of autistic traits.
