Résumé

Notre compréhension des expériences psychologiques de l’anxiété dans l’autisme pourrait être améliorée si elle était façonnée par des personnes autistes. Cependant, la recherche sur l’anxiété dans l’autisme commence souvent par des conceptualisations cognitives issues de personnes neurotypiques ou non autistes. Cette étude qualitative visait à explorer les expériences psychologiques de l’anxiété au moyen d’entretiens individuels semi-structurés avec un échantillon diversifié de 20 adultes autistes vivant au Royaume-Uni, âgés de 18 à 55 ans. Les transcriptions ont été analysées à l’aide de deux méthodes complémentaires : l’analyse conceptuelle de contenu, dans laquelle les théories existantes ont été testées par rapport aux données à l’aide de codes prédéfinis ; et l’analyse thématique réflexive, dans laquelle le sens a été construit à partir des données tout en tenant compte de la subjectivité de la recherche. Notre échantillon a fortement confirmé les construits cognitifs existants : l’intolérance à l’incertitude (100 % des participants), la pensée négative répétitive (100 %), les biais attentionnels (70 %) et l’imagerie mentale intrusive (70 %). Certaines caractéristiques spécifiques à l’autisme ont été indiquées. L’adhésion à l’imagerie est particulièrement frappante compte tenu des affirmations selon lesquelles l’autisme est souvent associé à une imagerie faible. Les adultes autistes ont décrit la manière dont le fait d’être autiste interagissait avec l’anxiété, à travers des expériences psychologiques et vécues. Le soutien existant pour les problèmes d’anxiété a été jugé insuffisant et pourrait être amélioré en affinant davantage notre compréhension de l’anxiété dans l’autisme grâce à de futures recherches quantitatives et qualitatives. Résumé grand public L’anxiété est une priorité majeure de recherche pour la communauté autiste. Nous pourrions accroître notre compréhension de ce qui peut contribuer aux problèmes d’anxiété dans l’autisme en parlant avec des personnes autistes et en apprenant les différentes expériences qu’elles ont vécues. Nous avons interrogé 20 adultes autistes vivant au Royaume-Uni, âgés de 18 à 55 ans, et leur avons posé des questions sur leurs problèmes d’anxiété, y compris sur ce qu’ils pensaient d’idées bien connues concernant ce qui pourrait être impliqué dans l’anxiété dans l’autisme. Nous avons examiné les réponses aux entretiens de deux façons. La première examine ce que les personnes autistes pensent des idées bien connues. La seconde examine les similitudes dans les réponses fournies par les personnes autistes. Les problèmes d’anxiété de tous les participants impliquaient des pensées négatives répétées et des difficultés face à des situations incertaines. La plupart ont également indiqué qu’ils avaient tendance à se concentrer sur les bruits ou d’autres expériences sensorielles dans leur environnement et à imaginer mentalement des situations lorsqu’ils étaient anxieux. Nous discutons des moyens par lesquels nous pourrions mieux comprendre l’anxiété dans l’autisme et améliorer le soutien apporté aux personnes autistes.

Our understanding of psychological experiences of anxiety in autism could be improved if it were shaped by autistic individuals. However, anxiety research in autism often begins with cognitive conceptualisations drawn from neurotypical or non-autistic individuals. This qualitative study aimed to explore psychological experiences of anxiety using semi-structured one-to-one interviews with a diverse sample of 20 U.K.-based, autistic adults, aged 18–55 years. Transcripts were analysed using two complementary methods: conceptual content analysis, where existing theories were tested against the data with predefined codes; and reflexive thematic analysis, where meaning was constructed from the data while considering research subjectivity. Our sample highly endorsed existing cognitive constructs: intolerance of uncertainty (100% of participants), repetitive negative thinking (100%), attentional biases (70%) and intrusive mental imagery (70%). Some autism-specific characteristics were indicated. The endorsement of imagery is particularly striking given claims that autism is often associated with weak imagery. Autistic adults described the way in which being autistic interacted with anxiety, through psychological and lived experiences. Existing support for anxiety problems was deemed insufficient and may be improved by further refining our understanding of anxiety in autism through future quantitative and qualitative research. Lay Abstract Anxiety is a top priority for research for the autistic community. We could increase our understanding of what might contribute to anxiety problems in autism by speaking to autistic people and learning about the different experiences they have had. We interviewed 20 U.K.-based autistic adults (aged 18–55 years) and asked them about their anxiety problems, including how they felt about well-known ideas of what might be involved in anxiety in autism. We looked at the answers to the interviews in two ways. The first way considers what autistic people think about the well-known ideas. The second way looks at similarities in the answers that autistic people gave. All participants’ anxiety problems involved repeated negative thoughts and struggling with uncertain situations. Most also said that they tend to focus on noises or other sensory experiences within their environment and imagine situations in their mind when they are anxious. Being autistic came with challenges that could make them more anxious, including struggling with changes to routine and being bullied. Autistic people described how anxiety made their life difficult and ways in which current support for anxiety is not helpful enough. We discuss ways that we could better understand anxiety in autism and improve the support given to autistic people.
