Résumé

Contexte : Un nombre croissant de personnes ayant reçu un diagnostic de trouble de la personnalité borderline (TPB) se demandent si elles présentent un autisme non diagnostiqué. Les recherches qualitatives antérieures ne se sont pas concentrées sur l’identification des obstacles et des facilitateurs de ce parcours diagnostique, n’ont pas exploré les perspectives des cliniciens et n’ont pas inclus les expériences des personnes qui ne savent pas si elles sont autistes ou non. Nous avons cherché à comprendre les perspectives de l’expérience vécue et des cliniciens concernant les facilitateurs et les obstacles à la reconnaissance d’un autisme non diagnostiqué chez les femmes et les personnes assignées de sexe féminin à la naissance (AFAB) ayant reçu un diagnostic de trouble de la personnalité. Méthodes : Nous avons mené des entretiens qualitatifs approfondis avec 15 cliniciens en santé mentale et 15 femmes/personnes AFAB ayant un diagnostic actuel ou antérieur de TPB et s’identifiant comme certainement ou possiblement autistes, provenant de l’ensemble du Royaume-Uni. Nous avons analysé les données d’entretien au moyen d’une analyse thématique réflexive. Résultats : Les participants cliniciens et ceux ayant une expérience vécue ont tous deux identifié de nombreux obstacles à la reconnaissance de l’autisme chez les femmes et les personnes AFAB ayant un diagnostic de TPB : diagnostics de TPB posés avec une évaluation minimale lors de crises de santé mentale, incitations systémiques à diagnostiquer un TPB afin d’accéder à des thérapies psychologiques, parcours de soins cloisonnés, réticence des cliniciens à remettre en question des diagnostics de TPB préexistants, pathologisation des patients qui remettent en question leur diagnostic de TPB, et manque de connaissances des cliniciens concernant les différentes présentations de l’autisme ou les manières dont l’autisme se manifeste de façon semblable ou différente du TPB. Les participants ont identifié de nombreuses manières dont les caractéristiques autistiques pouvaient être attribuées à tort aux symptômes du TPB, contribuant davantage à des diagnostics manqués ou erronés. Conclusion : Nos résultats suggèrent qu’une amélioration de la sensibilisation des cliniciens aux différentes présentations de l’autisme et au diagnostic différentiel avec le TPB est susceptible de réduire les erreurs diagnostiques, parallèlement à l’évitement de décisions diagnostiques rapides lors de crises de santé mentale. Notre étude souligne l’intérêt de rester ouvert à la remise en question des diagnostics préexistants, d’un travail conjoint entre les services consacrés à l’autisme et aux troubles de la personnalité, et de l’amélioration de l’accès transdiagnostique aux interventions psychologiques.

Background: Growing numbers of people with a borderline personality disorder (BPD) diagnosis are querying whether they have undiagnosed autism. Previous qualitative research has not focused on identifying barriers and facilitators to this diagnostic journey, has not explored the perspectives of clinicians, and has not included the experiences of people who are unsure whether they are autistic or not. We aimed to understand lived experience and clinician perspectives on facilitators and barriers to recognizing undiagnosed autism in women and people assigned female at birth (AFAB) with a diagnosis of personality disorder. Methods: We carried out in-depth qualitative interviews with 15 mental health clinicians and 15 women/AFAB people who had a current or prior diagnosis of BPD and identified as definitely or possibly autistic, from across the United Kingdom. We analyzed the interview data using reflexive thematic analysis. Results: Both clinician and lived experience participants identified many barriers to recognizing autism in women and AFAB people with a BPD diagnosis: BPD diagnoses being made with minimal assessment during mental health crises, systemic incentivization to diagnose BPD in order to access psychological therapies, siloed service pathways, clinician reluctance to question preexisting BPD diagnoses, pathologizing of patients for questioning their BPD diagnosis, and lack of clinician knowledge about different presentations of autism or about ways that autism presents similarly and differently to BPD. Participants identified numerous ways in which autistic characteristics could be misattributed as symptomatic of BPD, further contributing to missed or misdiagnosis. Conclusion: Our findings suggest that improving clinician awareness of different presentations of autism, and of differential diagnosis from BPD, is likely to reduce misdiagnosis, alongside avoiding rapid diagnostic decisions during mental health crises. Our study highlights the value of being open to questioning preexisting diagnoses, joint working across autism and personality disorder services, and improving transdiagnostic access to psychological interventions.
