Résumé L'anxiété est une affection concomitante fréquente chez les enfants autistes d'âge préscolaire. Bien que de nombreuses recherches aient évalué des interventions de prévention/réduction de l'anxiété chez les enfants d'âge préscolaire neurotypiques, ces recherches sont limitées chez les enfants autistes. Cinquante-sept parents d'enfants autistes âgés de 4 à 5 ans, présentant des niveaux d'anxiété variables, ont participé à un essai contrôlé randomisé d'une intervention médiée par les parents, spécifique à l'autisme (CLK-CUES), visant à prévenir ou réduire l'anxiété chez les enfants autistes d'âge préscolaire. Les évaluations initiales, à court terme (après l'intervention) et à plus long terme (suivi à 12 mois) comprenaient l'anxiété de l'enfant (ASC-ASD-P et PAS-R), l'intolérance à l'incertitude et le bien-être des parents. (Enregistrement de l'essai ACTRN12620001322921). Aucun effet nocif de l'intervention ou de l'essai n'a été signalé. Les modèles linéaires mixtes montrent une interaction groupe x temps significative pour le score total de l'ASC-ASD-P et la sous-échelle Incertitude, avec des tailles d'effet moyennes et importantes. Les analyses post-hoc montrent une diminution significative, uniquement dans le groupe d'intervention, de l'anxiété (plus précisément, l'anxiété liée à l'incertitude) entre avant et après l'intervention, maintenue au suivi à un an. Aucun changement n'a été observé pour la PAS-R ou les autres sous-échelles de l'ASC-ASD-P. CLK-CUES semble prometteur comme moyen de prévenir et de réduire l'anxiété chez les jeunes enfants autistes, en particulier l'anxiété liée à l'incertitude. Des essais avec des échantillons plus importants sont nécessaires. Les résultats soulignent également l'importance d'utiliser des mesures conçues pour les enfants autistes.

Abstract Anxiety is a common co-occurring condition for autistic preschoolers. Whilst there has been extensive research evaluating anxiety prevention/reduction interventions for neurotypical preschoolers, such research is limited for autistic children. Fifty-seven parents of autistic 4–5-year olds, with varying levels of anxiety, participated in a randomised controlled trial of an autism-specific, parent-mediated intervention (CLK-CUES) to prevent or reduce anxiety in autistic preschoolers. Baseline, short-term (post-intervention) and longer term (12 months follow-up) assessments included child anxiety (ASC-ASD-P and PAS-R), intolerance of uncertainty, and parent well-being. (Trial registration ACTRN12620001322921). There were no reports of harmful effects of the intervention or trial. Linear mixed models show a significant group x time interaction for ASC-ASD-P Total score and the Uncertainty subscale with medium and large effect sizes. Post-hoc analyses show a significant decline for only the intervention group in anxiety (specifically, anxiety around uncertainty) from pre-post intervention, maintained at one year follow-up. There was no change on the PAS-R or other ASC-ASD-P subscales. CLK-CUES shows promise as a way to prevent and reduce anxiety in young autistic children, specifically anxiety related to uncertainty. Trials with larger samples are warranted. Findings also highlight the importance of using measures designed for autistic children.
