Résumé

Les syndromes de sensibilité centrale (CSS) constituent un groupe d’affections de santé dont on pense qu’elles incluent une sensibilisation sous-jacente du système nerveux central. Des données suggèrent que les adultes autistes ont une santé physique plus mauvaise que la population générale et sont plus susceptibles d’avoir un CSS. Cette étude a examiné les diagnostics et les symptômes de CSS chez des adultes autistes et non autistes, afin de déterminer si les symptômes de CSS étaient liés aux traits autistiques, à la santé mentale, à la sensibilité sensorielle, à l’âge ou au genre. Les participants comprenaient 534 adultes ayant des diagnostics cliniques d’autisme, de CSS, des deux diagnostics ou d’aucun des deux (c’est-à-dire le groupe de comparaison), qui ont été recrutés par l’intermédiaire des réseaux sociaux, de groupes de soutien et d’affiliations institutionnelles. Les participants ont rempli en ligne des questionnaires validés d’autoévaluation, notamment l’Autism Spectrum Quotient (AQ), le Central Sensitization Inventory (CSI), le Sensory Perception Quotient (SPQ), ainsi que le PHQ-9 et le GAD-7. Les personnes autistes sans CSS diagnostiqué ont rapporté significativement plus de symptômes de CSS que le groupe de comparaison, avec un score moyen supérieur au seuil clinique. Les participants non autistes ayant un CSS présentaient significativement plus de traits autistiques que le groupe de comparaison. Les personnes autistes ayant un CSS ont rapporté la plus grande sensibilité sensorielle, les groupes autisme seul et CSS seul rapportant des niveaux similaires de sensibilité sensorielle, et tous les groupes diagnostiques rapportant une sensibilité sensorielle plus élevée que le groupe de comparaison. La sensibilité sensorielle, l’anxiété, les traits autistiques, l’âge et le genre étaient tous des prédicteurs significatifs des symptômes de CSS. Central sensitivity syndromes (CSSs) are a group of health conditions thought to include an underlying sensitisation of the central nervous system. Evidence suggests autistic adults experience poorer physical health than the general population and are more likely to have a CSS. This study examined CSS diagnoses and symptoms in autistic and non-autistic adults, to determine whether CSS symptoms were related to autistic traits, mental health, sensory sensitivity, age or gender. Participants included 534 adults with clinical diagnoses of autism, CSS, both diagnoses or neither (i.e., comparison group), who were recruited through social media, support groups and institutional affiliations. Participants completed online self-report validated questionnaires, including the Autism Spectrum Quotient (AQ), Central Sensitization Inventory (CSI), Sensory Perception Quotient (SPQ), and the PHQ-9 and GAD-7. Autistic people without a diagnosed CSS reported significantly more CSS symptoms than the comparison group, with a mean score above the clinical cut-off. Non-autistic participants with a CSS had significantly more autistic traits than the comparison group. Autistic people with a CSS reported the most sensory sensitivity, with autism only and CSS only groups reporting similar levels of sensory sensitivity and all diagnostic groups reporting more sensory sensitivity than the comparison group. Sensory sensitivity, anxiety, autistic traits, age and gender were all significant predictors of CSS symptoms. The overlap in symptoms between autistic individuals and those with CSS suggests diagnostic overshadowing and possible under-diagnosis or misdiagnosis. Furthermore, these symptoms may exacerbate or mask one another. Notwithstanding potential limitations of representativeness and selection bias, increased awareness of the association between autistic traits and CSS symptoms is important for clinicians to improve diagnostic accuracy and treatment.
