Résumé

Les sentiments émotionnels sont supposés être attribués à la représentation centrale des états corporels dans un contexte d’attente et d’incertitude concernant à la fois l’état interne et le monde extérieur. Les personnes neurodivergentes sont plus susceptibles de connaître des difficultés de santé mentale concomitantes, bien que les éclairages mécanistiques sous-tendant cette association soient rares. Nous avons donc entrepris une étude afin de déterminer si un traitement imprécis des signaux d’erreur proprioceptifs pourrait sous-tendre le lien entre la neurodivergence et la dysrégulation émotionnelle. Dans une cohorte de personnes présentant des affections chroniques complexes, notamment des douleurs chroniques et de la fatigue, ainsi que des traumatismes complexes, et dans un groupe de comparaison, nous avons évalué la présence de neurodivergence, de tissu conjonctif variant se manifestant par une hypermobilité articulaire, et de dysrégulation émotionnelle. Nous présentons un modèle conceptuel fondé sur les données montrant que le tissu conjonctif variant détermine si la surprise proprioceptive est liée à la dysrégulation émotionnelle chez les personnes neurodivergentes. Nous suggérons que les recherches futures dans ce domaine pourraient avoir d’importantes implications cliniques pour l’interaction entre le bien-être mental et physique chez les personnes neurodivergentes. Emotional feelings are putatively ascribed to central representation of bodily states in the context of expectation and uncertainty in both internal state and external world. Neurodivergent people are more likely to experience co-occurring mental health challenges, although mechanistic insights underpinning this association are scarce. We therefore undertook a study to test whether imprecise processing of proprioceptive error signals may underlie the connection between neurodivergence and emotional dysregulation. In a cohort of people with complex chronic conditions, including chronic pain/fatigue, and complex trauma, and in a comparison group, we assessed presence of neurodivergence, variant connective tissue manifested through joint hypermobility, and emotional dysregulation. We present a data-informed conceptual model showing that variant connective tissue determines whether proprioceptive surprise is linked with emotional dysregulation in neurodivergent individuals. We suggest that future research in this area may have important clinical implications for the interaction of mental and physical wellbeing in neurodivergent people.
