Contexte : L'existence et les besoins en soins de sante des personnes qui sont a la fois autistes et transgenres et non binaires (TNB) font de plus en plus l'objet de discussions publiques. Bien que les recherches montrant une prevalence plus elevee de l'autisme parmi les personnes TNB continuent de se developper, peu de travaux rendent compte de leurs experiences en matiere de soins primaires, de sante mentale et de soins d'affirmation de genre (GAC), en particulier entre les personnes TNB autistes auto-identifiees et diagnostiquees. Cet article explore cette intersection. Methodes : Nous avons realise une analyse de sous-groupe de l'enquete nationale Trans PULSE Canada 2019 (n = 2873). Nous avons inclus a la fois les personnes ayant declare elles-memes un diagnostic d'autisme (n = 230) et celles qui s'auto-identifiaient comme autistes sans en avoir un (n = 176). Nous avons compare les caracteristiques demographiques des participants, leur etat de sante et leurs experiences en matiere de soins de sante selon le statut autistique (diagnostique, auto-identifie uniquement, ou allistique [non autiste]) a l'aide de tests du chi carre ponderes et d'analyses de regression logistique. Resultats : Parmi les participants a Trans PULSE, 14,3 % etaient autistes (8,1 % diagnostiques, 6,2 % auto-identifies). Comparativement a leurs pairs allistiques, les participants autistes etaient plus jeunes, avaient des niveaux plus faibles d'education, d'emploi et de revenu, et etaient plus susceptibles de s'identifier comme asexuels. Ils ont egalement signale une moins bonne sante generale globale, un taux plus eleve de besoins en soins de sante non satisfaits et des disparites importantes en matiere de sante mentale. Bien que peu de participants autistes diagnostiques (3,7 %) ou auto-identifies (1,1 %) aient declare s'etre vu directement refuser des GAC en raison de l'autisme, 25,5 % des personnes diagnostiquees et 36,1 % des personnes auto-identifiees ont evite de maniere preventive de communiquer des informations a ce sujet lors d'evaluations de l'aptitude aux GAC au cours de l'annee precedente. Conclusions : Nos resultats soulignent la necessite de modifier la prise en charge des personnes TNB autistes dans les soins primaires et de sante mentale. Les recherches futures devraient explorer a la fois les reponses individuelles et les changements systematiques face a ces defis.
