Les personnes autistes connaissent de manière disproportionnée l’obésité, les maladies cardiovasculaires, le diabète et une série d’autres issues de santé défavorables, par rapport à la fois à la population générale et aux personnes présentant d’autres troubles du développement. Ces personnes connaissent également de manière disproportionnée le trouble de stress post-traumatique (TSPT). Bon nombre de ces affections apparaissent pendant l’adolescence et le début de l’âge adulte (âge de 15 à 30 ans). Cette étude a analysé les données de demandes de remboursement Medicaid (2008-2019) d’adolescents et de jeunes adultes autistes (n = 627,586 ; âge M = 17.15 [3.55]) et non autistes (n = 1,223,161 ; âge M = 19.35 [4.56]). À l’aide d’une régression logistique et en ajustant pour les caractéristiques démographiques et cliniques, cette étude : (1) a évalué les associations entre la présence d’autisme, d’obésité et d’autres comorbidités de santé à l’aide de l’Adolescent and Young Adult (AYA) Hope Comorbidity Index ; et (2) a testé le TSPT comme modérateur de ces associations. Par rapport aux bénéficiaires non autistes, les bénéficiaires autistes présentaient respectivement 2.12 (IC à 95 % : 2.09, 2.15) et 2.12 (IC à 95 % : 2.09, 2.16) fois plus de chances d’avoir une obésité et d’autres comorbidités de santé. Le TSPT a modéré ces associations de sorte que le statut d’autisme était plus fortement associé à l’obésité et aux comorbidités de santé chez les personnes sans diagnostic de TSPT que chez celles ayant un diagnostic de TSPT. Les adolescents et les jeunes adultes autistes connaissent des taux plus élevés d’obésité, de comorbidités de santé et de TSPT que leurs homologues non autistes. De futurs travaux sont nécessaires pour explorer la mesure du stress et du traumatisme au-delà des diagnostics de TSPT et élucider l’association précise entre le stress et le traumatisme et les issues de santé défavorables dans cette population.

Autistic individuals disproportionately experience obesity, cardiovascular disease, diabetes, and a range of other adverse health outcomes, relative to both the general population and those with other developmental conditions. These individuals also disproportionately experience Post-Traumatic Stress Disorder (PTSD). Many of these conditions emerge during adolescence and young adulthood (age 15-30). This study analyzed Medicaid claims data (2008-2019) from autistic (n = 627,586; M age = 17.15 [3.55]) and non-autistic (n = 1,223,161; M age 19.35 [4.56]) adolescent and young adults. Using logistic regression and adjusting for demographic and clinical characteristics, this study: (1) evaluated associations between the presence of autism, obesity, and other health co-morbidities using the Adolescent and Young Adult (AYA) Hope Comorbidity Index; and (2) tested PTSD as a moderator in these associations. Compared with non-autistic beneficiaries, autistic beneficiaries demonstrated 2.12 (95% CI: 2.09, 2.15) and 2.12 (95% CI: 2.09, 2.16) times the odds of having obesity and other health comorbidities, respectively. PTSD moderated these associations such that autism status was more strongly associated with obesity and health co-morbidities among those without a PTSD diagnosis compared to those with a PTSD diagnosis. Autistic adolescents and young adults experience higher rates of obesity, health co-morbidities, and PTSD relative to their non-autistic counterparts. Future work is needed to explore measurement of stress and trauma beyond PTSD diagnoses and elucidate the precise association between stress and trauma with adverse health outcomes in this population.
