Résumé

Les études antérieures sur l'hyperréactivité auditive dans l'autisme n'ont souvent pas distingué les différents aspects de l'inconfort sensoriel auditif, tels que l'hyperacousie et la misophonie. La présente étude caractérise rigoureusement plusieurs formes d'hyperréactivité sensorielle auditive, à l'aide d'une combinaison de tâches psychoacoustiques réalisées en laboratoire et de questionnaires auto-rapportés et rapportés par les aidants, et examine les différences entre groupes ainsi que les relations entre différentes mesures. Dix-huit adolescents autistes et 22 adolescents du groupe de comparaison ont évalué le caractère agréable de sons conventionnellement agréables, conventionnellement désagréables et de sons déclencheurs misophoniques, à des intensités de 50 à 80 dB SPL. Ils ont également fourni des seuils auditifs, des niveaux d'inconfort liés à l'intensité sonore et ont rempli plusieurs questionnaires ; leurs aidants ont rempli des questionnaires supplémentaires. Les participants autistes ont évalué les sons conventionnellement agréables comme étant plus désagréables à des intensités de 60 à 80 dB, mais l'inconfort global lié à l'intensité sonore causé par des sons purs ne différait pas significativement entre les groupes, et les évaluations des déclencheurs misophoniques courants ne semblaient pas non plus différer. Les participants autistes ont néanmoins rapporté eux-mêmes une hyperréactivité auditive plus importante, y compris une hyperacousie et une misophonie. Toutefois, dans le groupe autiste, les scores d'hyperréactivité auditive auto-rapportés étaient, de manière inattendue, inférieurs aux scores rapportés par les aidants. Les mesures issues des tâches psychoacoustiques et certaines mesures de la misophonie fondées sur des questionnaires concordaient bien, mais les mesures de l'hyperacousie ne concordaient pas bien. L'intolérance aux sons aurait affecté la qualité de vie des participants autistes. Toutefois, les résultats variés selon les différentes mesures, telles que les évaluations de sons purs par rapport aux sons conventionnellement agréables, soulignent la complexité et l'apparente idiosyncrasie de l'inconfort et de la détresse sensoriels dans l'autisme. Les tâches psychoacoustiques d'évaluation des sons ont montré une certaine convergence et une certaine divergence avec les mesures par questionnaire, soulignant l'importance d'une mesure multimodale.

Prior studies of auditory hyperreactivity in autism have often not distinguished between different aspects of auditory sensory discomfort, such as hyperacusis and misophonia. The present study rigorously characterises multiple forms of auditory sensory hyperreactivity, using a mixture of laboratory-based psychoacoustic tasks and self- and caregiver-reported questionnaires, and examines group differences and relationships among different measures. 18 autistic and 22 comparison adolescents rated the pleasantness of conventionally-pleasant, conventionally-unpleasant, and misophonic trigger sounds at intensities of 50 through 80 dB SPL. They also provided hearing thresholds, loudness discomfort levels, and filled out a number of questionnaires; their caregivers completed additional questionnaires. Autistic participants rated conventionally-pleasant sounds as being more unpleasant at intensities of 60 through 80 dB, but overall loudness discomfort caused by pure tones did not significantly differ across groups, nor did ratings of common misophonic triggers appear to differ. Autistic participants nevertheless self-reported more auditory hyper-reactivity, including hyperacusis and misophonia. However, in the autism group, self-reported auditory hyper-reactivity scores were unexpectedly lower than caregiver-reported scores. Psychoacoustic task and some questionnaire-based measures of misophonia converged well, but hyperacusis measures did not converge well. Sound intolerance reportedly affected autistic participants’ quality of life. However, varied findings with different measures, such as ratings of pure tones versus conventionally-pleasant sounds, emphasise the complexity and seeming idiosyncrasy of sensory discomfort and distress in autism. Psychoacoustic sound rating tasks showed some convergence and some divergence with questionnaire measures, pointing to the importance of multimodal measurement.
