Résumé

Un accès inéquitable à la surveillance développementale précoce de l’autisme est manifeste à l’échelle mondiale. Toutefois, peu de recherches examinent le diagnostic d’autisme, l’origine ethnique et les profils communautaires dans le cadre de la participation à des recherches visant l’identification précoce de l’autisme. Nous avons cherché à comprendre les relations entre l’origine ethnique de l’enfant, les caractéristiques démographiques maternelles et le diagnostic d’autisme, en comparant des données rétrospectives du recensement de 2016 pour huit zones d’administration locale (LGA) dans l’État de Victoria, en Australie. Des infirmières de santé maternelle et infantile (MCH) ont suivi 13 511 enfants de moins de 42 mois afin de détecter les signes précoces d’autisme au moyen des outils Social Attention Communication Surveillance-Revised (SACS-R) et SACS-R Preschool (SACS-PR), lors de consultations de suivi de l’enfant en bonne santé. Parmi eux, 340 enfants présentant une « forte probabilité » d’autisme ont participé à des évaluations développementales. L’origine ethnique maternelle des participants (« origine ethnique maternelle européenne », EME ; « origine ethnique maternelle non européenne », N-EME ; « origine ethnique maternelle mixte », MME), les facteurs socioéconomiques et la prévalence de l’autisme ont été comparés à ceux de leur communauté LGA. Les résultats ont indiqué que les participants à l’étude étaient représentatifs de leurs communautés LGA, bien que le bilinguisme et le multilinguisme aient été plus élevés dans notre cohorte. Des différences concernant l’emploi maternel actuel, le niveau d’instruction maternel, le revenu familial annuel et la prévalence de l’autisme ont été observées entre les groupes N-EME, EME et MME. Notre étude a montré que la participation à la recherche était déterminée par le niveau d’instruction maternel, l’emploi maternel et le revenu familial annuel, et que des recherches supplémentaires sont nécessaires pour comprendre ces relations.

Inequitable access to early autism developmental surveillance is evident globally. However, there is limited research examining autism diagnosis, ethnicity, and community profiles when engaging in research for the early identification of autism. We aimed to understand the relationships between child ethnicity, maternal demographics, and autism diagnosis, comparing retrospective data from the 2016 census for eight local government areas (LGAs) in Victoria, Australia. Maternal and child health (MCH) nurses monitored 13,511 children under 42 months for the early signs of autism using the Social Attention Communication Surveillance-Revised (SACS-R) and SACS-R Preschool (SACS-PR) tools during well-child checks. Of these, 340 children with a “high likelihood” of autism attended developmental assessments. Participants’ maternal ethnicity (‘European maternal ethnicity’, EME; ‘non-European maternal ethnicity,’ N-EME; ‘mixed maternal ethnicity,’ MME’), socioeconomic factors, and autism prevalence were compared to their LGA community. Results indicated that study participants were representative of their LGA communities, though bi- and multilingualism was higher in our cohort. Differences in current maternal employment, maternal education, annual family income, and autism prevalence were found between the N-EME, EME, and MME groups. Our study found that research engagement was driven by maternal education, maternal employment, and annual family income, and further research is required to understand these relationships.
