Résumé

OBJECTIFS : Étudier la fréquence et l’âge au diagnostic du trouble du spectre de l’autisme (TSA) chez les enfants âgés de moins de 7 ans vivant en Australie. PLAN D’ÉTUDE ET PARTICIPANTS : Analyse de données désidentifiées concernant 15 074 enfants âgés de moins de 7 ans inscrits au Helping Children with Autism Package (HCWAP ; un programme qui fournit des financements pour l’accès à des services d’intervention précoce et de soutien dans toute l’Australie) entre le 1er juillet 2010 et le 30 juin 2012. PRINCIPALES MESURES DES RÉSULTATS : Âge au diagnostic de TSA tel que confirmé par un pédiatre, un psychiatre et/ou une évaluation par une équipe multidisciplinaire. RÉSULTATS : L’âge moyen au diagnostic de TSA chez les enfants inscrits au HCWAP est actuellement de 49 mois, l’âge le plus fréquemment rapporté étant de 71 mois. Des différences d’âge au diagnostic étaient évidentes entre les États, les enfants d’Australie-Occidentale et de Nouvelle-Galles du Sud étant diagnostiqués à un âge plus jeune. Dans l’ensemble de l’Australie, 0,74 % de la population d’enfants âgés de moins de 7 ans ont actuellement reçu un diagnostic de TSA et sont inscrits au HCWAP. Une proportion plus élevée d’enfants était inscrite au HCWAP dans l’État de Victoria par rapport aux autres États. Aucune différence d’âge au diagnostic n’a été observée entre les Australiens autochtones et non autochtones, mais les enfants issus d’un contexte culturellement et linguistiquement diversifié étaient diagnostiqués 5 mois plus tôt que les autres enfants. CONCLUSIONS : Il pourrait exister un écart substantiel entre l’âge auquel un diagnostic fiable et précis de TSA est possible et l’âge moyen auquel les enfants reçoivent actuellement un diagnostic. La fréquence des diagnostics de TSA en Australie a considérablement augmenté par rapport aux estimations publiées antérieurement.

OBJECTIVES: To investigate the frequency and age at diagnosis of autism spectrum disorder (ASD) in children aged under 7 years living in Australia. DESIGN AND PARTICIPANTS: Analysis of de-identified data on 15 074 children aged under 7 years registered with the Helping Children with Autism Package (HCWAP; a program that provides funding for access to early intervention and support services throughout Australia) between 1 July 2010 and 30 June 2012. MAIN OUTCOME MEASURES: Age at diagnosis of ASD as confirmed by a paediatrician, psychiatrist and/or multidisciplinary team assessment. RESULTS: The average age at diagnosis of ASD in children registered with the HCWAP is currently 49 months, with the most frequently reported age being 71 months. Differences were evident in age at diagnosis across states, with children in Western Australia and New South Wales being diagnosed at a younger age. Across Australia, 0.74% of the population of children aged under 7 years are currently diagnosed with ASD and registered with the HCWAP. A higher proportion of children were registered with the HCWAP in Victoria compared with other states. There was no difference in age at diagnosis between Indigenous and non-Indigenous Australians, but children from a culturally and linguistically diverse background were diagnosed 5 months earlier than other children. CONCLUSIONS: There may be a substantial gap between the age at which a reliable and accurate diagnosis of ASD is possible and the average age that children are currently diagnosed. The frequency of ASD diagnoses in Australia has increased substantially from previously published estimates.
