Résumé

OBJECTIFS : L'hyperacousie, définie comme une tolérance diminuée aux sons à des niveaux qui ne gêneraient pas la plupart des personnes, est fréquemment observée chez les personnes présentant un trouble du spectre de l'autisme (TSA). Malgré l'altération fonctionnelle attribuable à l'hyperacousie, on sait peu de choses sur sa prévalence ou son histoire naturelle dans la population TSA. L'objectif de cette étude était de réaliser une revue systématique et une méta-analyse estimant la prévalence actuelle et au cours de la vie de l'hyperacousie chez les enfants, les adolescents et les adultes présentant un TSA. En estimant précisément le fardeau de l'hyperacousie dans la population TSA, la présente étude vise à améliorer la reconnaissance de ce symptôme particulier du TSA et à souligner la nécessité de recherches supplémentaires sur les causes, la prévention et le traitement de l'hyperacousie chez les personnes appartenant au spectre. MÉTHODES : Nous avons effectué des recherches dans PubMed et ProQuest afin d'identifier des articles évalués par les pairs publiés en anglais après janvier 1993. Nous avons également réalisé des recherches ciblées dans Google Scholar et la littérature grise, y compris des études publiées jusqu'en mai 2020. Les études admissibles comprenaient au moins 20 personnes de tout âge ayant reçu un diagnostic de TSA et rapportaient des données à partir desquelles la proportion de personnes avec TSA présentant une hyperacousie actuelle et/ou au cours de la vie pouvait être dérivée. Afin de tenir compte de multiples estimations de prévalence dérivées des mêmes échantillons, nous avons utilisé des méta-analyses bayésiennes à effets aléatoires à trois niveaux pour estimer la prévalence actuelle et au cours de la vie de l'hyperacousie. Une méta-régression bayésienne a été utilisée pour évaluer les modérateurs potentiels de la prévalence actuelle de l'hyperacousie. Afin de réduire l'hétérogénéité due aux définitions variables de l'hyperacousie, nous avons effectué une analyse de sensibilité sur le sous-ensemble d'études ayant déterminé le statut d'hyperacousie à l'aide de l'Autism Diagnostic Interview-Revised (ADI-R), un entretien structuré avec les parents. RÉSULTATS : Au total, 7783 articles non dupliqués ont été examinés, dont 67 ont été inclus dans la revue et la synthèse. Le statut d'hyperacousie a été déterminé de multiples façons selon les études, 60 articles utilisant des entretiens ou des questionnaires et sept utilisant des observations comportementales ou des mesures objectives. L'âge moyen (étendue) des échantillons dans les études incluses était de 7,88 ans (1,00 à 34,89 ans). La méta-analyse des mesures par entretien/questionnaire (k(3) = 103, nASD = 13 093) a estimé la prévalence actuelle et au cours de la vie de l'hyperacousie dans le TSA à 41,42 % (ICr à 95 %, 37,23 à 45,84 %) et 60,58 % (50,37 à 69,76 %), respectivement. Une analyse de sensibilité limitée aux estimations de prévalence dérivées de l'ADI-R (k(3) = 25, nASD = 5028) a produit des valeurs similaires. L'estimation de la prévalence actuelle de l'hyperacousie à l'aide de mesures objectives/d'observation (k(3) = 8, nASD = 488) était de 27,30 % (14,92 à 46,31 %). L'hétérogénéité dans l'échantillon complet de mesures par entretien/questionnaire était importante, mais n'était expliquée de manière significative par aucun modérateur testé. Toutefois, la prévalence augmentait fortement avec l'âge dans les études utilisant l'ADI-R (BF10 = 93,10, R2Het = 0,692). CONCLUSIONS : Dans cette méta-analyse, nous avons constaté une prévalence élevée de l'hyperacousie actuelle et au cours de la vie chez les personnes présentant un TSA, la majorité des personnes appartenant au spectre de l'autisme connaissant une hyperacousie à un moment ou à un autre de leur vie. La prévalence élevée de l'hyperacousie chez les personnes présentant un TSA tout au long de la vie souligne la nécessité de recherches supplémentaires sur la tolérance aux sons dans cette population ainsi que du développement de services et/ou d'interventions visant à réduire le fardeau de ce symptôme fréquent.

OBJECTIVES: Hyperacusis, defined as decreased tolerance to sound at levels that would not trouble most individuals, is frequently observed in individuals with autism spectrum disorder (ASD). Despite the functional impairment attributable to hyperacusis, little is known about its prevalence or natural history in the ASD population. The objective of this study was to conduct a systematic review and meta-analysis estimating the current and lifetime prevalence of hyperacusis in children, adolescents, and adults with ASD. By precisely estimating the burden of hyperacusis in the ASD population, the present study aims to enhance recognition of this particular symptom of ASD and highlight the need for additional research into the causes, prevention, and treatment of hyperacusis in persons on the spectrum. DESIGN: We searched PubMed and ProQuest to identify peer-reviewed articles published in English after January 1993. We additionally performed targeted searches of Google Scholar and the gray literature, including studies published through May 2020. Eligible studies included at least 20 individuals with diagnosed ASD of any age and reported data from which the proportion of ASD individuals with current and/or lifetime hyperacusis could be derived. To account for multiple prevalence estimates derived from the same samples, we utilized three-level Bayesian random-effects meta-analyses to estimate the current and lifetime prevalence of hyperacusis. Bayesian meta-regression was used to assess potential moderators of current hyperacusis prevalence. To reduce heterogeneity due to varying definitions of hyperacusis, we performed a sensitivity analysis on the subset of studies that ascertained hyperacusis status using the Autism Diagnostic Interview-Revised (ADI-R), a structured parent interview. RESULTS: A total of 7783 nonduplicate articles were screened, of which 67 were included in the review and synthesis. Hyperacusis status was ascertained in multiple ways across studies, with 60 articles employing interviews or questionnaires and seven using behavioral observations or objective measures. The mean (range) age of samples in the included studies was 7.88 years (1.00 to 34.89 years). The meta-analysis of interview/questionnaire measures (k(3) = 103, nASD = 13,093) estimated the current and lifetime prevalence of hyperacusis in ASD to be 41.42% (95% CrI, 37.23 to 45.84%) and 60.58% (50.37 to 69.76%), respectively. A sensitivity analysis restricted to prevalence estimates derived from the ADI-R (k(3) = 25, nASD = 5028) produced similar values. The estimate of current hyperacusis prevalence using objective/observational measures (k(3) = 8, nASD = 488) was 27.30% (14.92 to 46.31%). Heterogeneity in the full sample of interview/questionnaire measures was substantial but not significantly explained by any tested moderator. However, prevalence increased sharply with increasing age in studies using the ADI-R (BF10 = 93.10, R2Het = 0.692). CONCLUSIONS: In this meta-analysis, we found a high prevalence of current and lifetime hyperacusis in individuals with ASD, with a majority of individuals on the autism spectrum experiencing hyperacusis at some point in their lives. The high prevalence of hyperacusis in individuals with ASD across the lifespan highlights the need for further research on sound tolerance in this population and the development of services and/or interventions to reduce the burden of this common symptom.
