Résumé

Introduction Les taux de prévalence des problèmes émotionnels et comportementaux (PEC) chez les enfants et les jeunes autistes sont élevés (40–70 %) et entraînent souvent une atteinte sévère et chronique. En outre, les enfants autistes sont également plus susceptibles de connaître une adversité familiale « socio-écologique » que leurs pairs au développement neurotypique, notamment l'isolement social, la maltraitance des enfants, la maladie mentale des personnes qui s'occupent d'eux et le risque socioéconomique. Ces facteurs de stress familiaux augmentent le risque de PEC concomitants chez les enfants autistes et peuvent souvent entraver l'accès à des soins fondés sur des données probantes, amplifiant ainsi les inégalités de santé à long terme pour les enfants autistes et les personnes qui s'occupent d'eux. Dans le paysage actuel des services d'autisme, il existe peu de programmes évolutifs fondés sur des données probantes qui répondent adéquatement à ces besoins. Le Family Check-Up (FCU®) est une intervention brève, axée sur les forces et adaptée, centrée sur la famille, qui soutient une parentalité positive et évalue explicitement les déterminants sociaux de la santé mentale de l'enfant et de la famille dans un cadre écologique. Des études ont démontré des résultats positifs à long terme pour les enfants et les personnes qui s'occupent d'eux dans d'autres populations, mais le FCU® n'a pas été évalué auprès de familles d'enfants et de jeunes autistes. Nous avons donc cherché à évaluer la mise en œuvre du FCU® au sein d'un programme d'autisme établi et financé par des fonds publics en Ontario, au Canada, avec une prestation par des thérapeutes en autisme, afin de démontrer une efficacité durable dans des contextes réels. Méthodes Dans cette étude, nous présentons le protocole d'une approche hybride de mise en œuvre et d'efficacité comportant deux composantes principales : (1) un essai contrôlé randomisé à bras parallèles portant sur N = 80 enfants/jeunes autistes (âgés de 6 à 17 ans) présentant des niveaux élevés de PEC, ainsi que leurs personnes qui s'occupent d'eux. Les résultats principaux et secondaires comprennent les PEC de l'enfant, ainsi que le bien-être et la parentalité des personnes qui s'occupent de lui. (2) Une étude de mise en œuvre à méthodes mixtes, visant à décrire les facilitateurs et les obstacles à la mise en œuvre du FCU® dans un contexte de services d'autisme. Discussion Les interventions évolutives centrées sur la famille et axées sur l'écologie sont prometteuses en tant qu'éléments clés d'un cadre de santé publique visant à réduire les inégalités en matière de santé mentale parmi les enfants, les jeunes autistes et les personnes qui s'occupent d'eux. Les résultats de cette étude éclaireront le perfectionnement ultérieur du programme et son déploiement à plus grande échelle.

Introduction Prevalence rates of emotional and behavior problems (EBP) in autistic children and youth are high (40–70%), and often cause severe and chronic impairment. Furthermore, autistic children are also more likely to experience family “social-ecological” adversity compared to neurotypically developing peers, including social isolation, child maltreatment, caregiver mental illness, and socioeconomic risk. These family stressors increase the risk of co-occurring EBP among autistic children and can often impede access to evidence-based care, thus amplifying long-term health inequities for autistic children and their caregivers. In the current autism services landscape, there are few scalable, evidence-based programs that adequately address these needs. The Family Check-Up (FCU®) is a brief, strength-based, and tailored family-centered intervention that supports positive parenting and explicitly assesses the social determinants of child and family mental health within an ecological framework. Studies have demonstrated long-term positive child and caregiver outcomes in other populations, but the FCU® has not been evaluated in families of autistic children and youth. Therefore, we aimed to evaluate FCU® implementation within an established, publicly funded Autism Program in Ontario, Canada, with delivery by autism therapists, to demonstrate sustainable effectiveness within real-world settings. Methods In this study, we outline the protocol for a hybrid implementation-effectiveness approach with two key components: (1) A parallel-arm randomized controlled trial of N = 80 autistic children/youth (ages 6–17 years) and high levels of EBP and their caregivers. Primary and secondary outcomes include child EBP, and caregiver well-being and parenting. (2) A mixed methods implementation study, to describe facilitators and barriers to implementation of the FCU® within an autism service setting. Discussion Scalable, ecologically focused family-centered interventions offer promise as key components of a public health framework aimed at reducing mental health inequities among autistic children, youth, and their caregivers. Results of this study will inform further program refinement and scale-up.
