Résumé
Les besoins de la population noire/afro-americaine aux Etats-Unis ont historiquement ete negliges dans la recherche, y compris dans le developpement de therapies et de services visant a soutenir les personnes autistes. L'etendue des interventions visant a soutenir les personnes autistes qui sont concues ou adaptees afin d'etre culturellement adaptees a la population noire americaine n'est pas connue. L'etat de ces interventions abordant systematiquement les facteurs culturels est egalement peu explore. La presente revue systematique a examine les donnees probantes issues de la recherche sur les interventions relatives a l'autisme soit concues, soit adaptees pour les personnes noires aux Etats-Unis. La recherche ne comportait pas de plage de dates specifique, mais a identifie de la litterature a partir de 1999, et la recherche s'est terminee en 2023. Au total, huit etudes ont ete identifiees et examinees : quatre etaient des interventions concues specifiquement pour la population noire autiste, une etait une intervention adaptee, et trois etaient des interventions evaluees aupres d'un echantillon noir autiste. Pour evaluer ces etudes, nous avons utilise la Cultural Adaptation Checklist et avons constate que peu d'interventions relatives a l'autisme sont systematiquement adaptees pour les personnes noires. Les chercheurs sur l'autisme qui ont mobilise la population noire autiste ou leurs familles ont fait preuve de reflexion dans leurs manieres de les mobiliser ; toutefois, aucun cadre universel n'a guide les efforts de developpement ou d'adaptation de ces interventions. Le developpement d'un plus grand nombre d'interventions relatives a l'autisme davantage adaptees sur le plan culturel (c'est-a-dire des interventions adaptees fondees sur des donnees probantes) pour la population noire est prometteur pour des pratiques relatives a l'autisme plus equitables. Resume grand public Les personnes noires/afro-americaines aux Etats-Unis qui ont un diagnostic d'autisme connaissent souvent des inegalites liees aux services, notamment le fait de ne pas avoir le meme acces a des soins de haute qualite pour l'autisme et les troubles associes (par exemple, des interventions comportementales), et sont moins susceptibles de maintenir un engagement durable dans le traitement tout au long de leur vie. Bien que les interventions visant a soutenir les personnes autistes soient generalement concues pour etre universelles, on craint que le fait que ces interventions ne soient pas adaptees a la population noire/afro-americaine puisse reduire leur impact global en raison d'un manque de prise en compte des besoins des enfants noirs ou des familles qui recoivent l'intervention. La presente revue systematique a resume les recherches sur les interventions elaborees pour la communaute noire de l'autisme, y compris les enfants noirs autistes et leurs aidants. Apres une recherche exhaustive et systematique, huit publications evaluees par les pairs ont ete identifiees comme repondant aux criteres d'inclusion de l'etude. La majorite des interventions etaient adaptees aux aidants noirs d'enfants autistes. Les chercheurs sur l'autisme demontrent differentes strategies pour mobiliser les aidants noirs de manieres culturellement adaptees ; toutefois, des recherches supplementaires sur ces interventions sont necessaires afin d'evaluer leur efficacite. En outre, il existe encore peu d'interventions adaptees afin d'etre culturellement adaptees aux personnes autistes noires/afro-americaines. Le cadre de la Cultural Adaptation Checklist est une approche nouvelle susceptible de devenir la norme pour adapter les interventions aux besoins de groupes culturellement divers. L'adaptation culturelle est une dimension importante dans le developpement d'interventions qui produisent des resultats optimaux pour l'eventail de la diversite aux Etats-Unis et constitue une etape importante vers l'atteinte de pratiques de recherche sur l'autisme equitables.
