Résumé

Introduction : Le trouble du spectre de l'autisme (TSA) comprend des difficultés de communication sociale ainsi que des comportements restrictifs et répétitifs. Malgré une prévalence mondiale accrue, peu de choses restent connues sur la détection et le diagnostic du TSA en Chine continentale. Notre objectif était de mener une étude pilote de la surveillance de l'attention sociale et de la communication à Tianjin, Chine (SACS-C), afin d'identifier précocement le TSA par des professionnels, en comparaison avec la Checklist for Autism in Toddlers-23 (CHAT-23), remplie par les parents et les professionnels. Méthodes : Au total, 10 514 enfants ont été suivis au Tianjin Women and Children's Health Centre (TWCHC) au moyen du SACS-C entre 12 et 24 mois, après une formation d'une demi-journée dispensée à 225 praticiens de santé infantile. Les enfants considérés comme présentant une « forte probabilité » de TSA selon le SACS, la CHAT-23, ou les deux, ont été orientés vers des évaluations développementales au TWCHC. Résultats : Au total, 87 enfants (0,8 %) ont été identifiés comme présentant une « forte probabilité » selon le SACS-C, dont 57 ont été évalués pour le TSA (66 %) ; 24 enfants ont ensuite reçu un diagnostic de TSA (42,1 %), et les 33 autres (57,9 %) ont reçu un diagnostic de retards du développement et/ou du langage. Le SACS-C présentait une valeur prédictive positive (VPP) de 42,1 %, une valeur prédictive négative (VPN) de 99,8 %, ainsi qu'une sensibilité et une spécificité de 53,3 % et 99,7 %, respectivement. Seuls 21 enfants ont été identifiés comme présentant un « risque élevé » de TSA selon la CHAT-23 (0,2 %), dont 14 ont été évalués pour le TSA (66 %) ; neuf ont reçu un diagnostic de TSA (64,3 %) et les cinq autres enfants ont reçu un diagnostic de retards du développement et/ou du langage. La CHAT-23 présentait une VPP globale de 64,3 %, une VPN de 99,6 %, une sensibilité de 27,3 % et une spécificité de 99,9 %. Conclusion : Il s'agissait de la première étude à grande échelle identifiant le TSA chez des enfants âgés de 12 à 24 mois à Tianjin, Chine. Nous avons établi la faisabilité de former des praticiens de santé communautaire à surveiller les nourrissons et les jeunes enfants afin de détecter les signes précoces de TSA, et déterminé l'efficacité du SACS-C. En outre, le SACS-C présentait un meilleur équilibre entre exactitude et sensibilité dans la détection du TSA, la CHAT-23 ne détectant pas la majorité des enfants du spectre (72,7 %) contre le SACS-C (46,7 %). Toutefois, des travaux supplémentaires sont nécessaires pour améliorer les propriétés psychométriques de l'utilisation du SACS-C en Chine continentale afin de la rendre comparable à son utilisation en Australie.

Introduction: Autism spectrum disorder (ASD) comprises difficulties in social communication and restrictive and repetitive behaviours. Despite an increased global prevalence, little remains known about detection and diagnosis of ASD in mainland China. Our aim was to conduct a pilot investigation of Social Attention and Communication Surveillance in Tianjin, China (SACS-C) to identify ASD early by professionals, compared to the Checklist for Autism in Toddlers-23 (CHAT-23) completed by parents and professionals. Methods: A total of 10,514 children were monitored at the Tianjin Women and Children’s Health Centre (TWCHC) on the SACS-C at 12 - 24 months of age following a half-day training of 225 child health practitioners. Children deemed at “high likelihood” for ASD on either the SACS, CHAT-23, or both, were referred for developmental assessments at TWCHC. Results: A total of 87 children (0.8%) were identified at “high likelihood” on the SACS-C, of whom 57 were assessed for ASD (66%); 24 children were subsequently diagnosed with ASD (42.1%), and the remaining 33 (57.9%) were diagnosed with developmental and/or language delays. The SACS-C had a positive predictive value (PPV) of 42.1%, a negative predictive value (NPV) of 99.8%, and sensitivity and specificity of 53.3% and 99.7%, respectively. Only 21 children were identified at “high risk” for ASD on the CHAT-23 (.2%), with 14 children assessed for ASD (66%); nine were diagnosed with ASD (64.3%) and the remaining five children were diagnosed with developmental and/or language delays. The CHAT-23 had an overall PPV of 64.3%, NPV of 99.6%, sensitivity of 27.3%, and specificity of 99.9%. Conclusion: This was the first large-scale study identifying ASD in 12-24-month-old children in Tianjin, China. We ascertained the feasibility of training community health practitioners to monitor infants and toddlers for the early signs of ASD, and determined the effectiveness of SACS-C. Furthermore, the SACS-C had a better balance between accuracy and sensitivity in detecting ASD, with the CHAT-23 missing the majority of children on the spectrum (72.7%) vs the SACS-C (46.7%). However, more work is needed to improve the psychometric properties in using the SACS-C in mainland China to make it comparable to its use in Australia.
