Contexte : La détection précoce de troubles du développement tels que l'autisme est essentielle pour garantir que les enfants puissent accéder à des soutiens, services et interventions appropriés, opportuns et fondés sur des données probantes. Les enfants qui présentent des troubles du développement non détectés au début de leur vie sont plus susceptibles de développer ultérieurement des problèmes de santé, de développement, d'apprentissage et de comportement, lesquels peuvent à leur tour avoir un effet cumulatif tout au long de la vie. Méthodes : Le présent protocole décrit un essai contrôlé randomisé en grappes, multisite, comparant une filière de surveillance développementale de l'autisme aux soins habituels, en utilisant des consultations opportunistes auprès de médecins généralistes (GP). Les unités de randomisation sont des cliniques de médecine générale réparties dans deux États australiens, la Nouvelle-Galles du Sud et le Victoria, avec trente cliniques dans chaque État, chacune visant à recruter environ quarante enfants âgés d'environ 18 à 24 mois, pour un total d'environ 2 400 participants. Les enfants seront randomisés dans deux grappes, à savoir une filière de surveillance de l'autisme (ASP) ou une surveillance habituelle (SaU). Le processus de dépistage dans le bras ASP comprend des dépistages primaires et secondaires des préoccupations développementales liées à l'autisme, en utilisant à la fois les signalements et les observations des parents et des GP. Les enfants des deux bras qui présentent des signes de préoccupations développementales liées à l'autisme se verront proposer une évaluation développementale complète par l'équipe de recherche à l'âge de 24 mois afin de déterminer l'efficacité de la surveillance développementale pour identifier avec succès les enfants autistes. Enregistrement de l'essai : L'essai est enregistré auprès de l'ANZCTR (ACTRN12619001200178) et la communication des résultats de l'essai suivra les recommandations de la déclaration CONSORT.
