Résumé

La Social Attention and Communication Study a impliqué la mise en œuvre réussie d'une surveillance du développement des marqueurs précoces des troubles du spectre de l'autisme dans un contexte communautaire. L'objectif de la présente étude était de déterminer les marqueurs les plus discriminants et prédictifs des troubles du spectre de l'autisme utilisés dans la Social Attention and Communication Study à 12, 18 et 24 mois d'âge, afin qu'ils puissent être utilisés pour identifier avec une plus grande précision les enfants présentant des troubles du spectre de l'autisme. Le pourcentage de réponses « oui/non » pour chaque marqueur comportemental a été comparé entre des enfants présentant un trouble autistique (n = 39), un trouble du spectre de l'autisme (n = 50) et un retard du développement et/ou du langage (n = 20) de 12 à 24 mois, une régression logistique ayant également été effectuée à 24 mois. À tous les âges, les principaux marqueurs récurrents du trouble autistique et du trouble du spectre de l'autisme étaient des déficits de contact visuel et de pointage et, à partir de 18 mois, des déficits de montrer sont devenus un marqueur important. En combinaison, ces comportements, ainsi que le jeu de faire semblant, se sont révélés constituer le meilleur groupe de prédicteurs pour une classification diagnostique par meilleure estimation de trouble autistique/trouble du spectre de l'autisme à 24 mois. Il est avancé que les marqueurs identifiés devraient être surveillés de façon répétée au cours de la deuxième année de vie par des professionnels de santé communautaires.

The Social Attention and Communication Study involved the successful implementation of developmental surveillance of the early markers of autism spectrum disorders in a community-based setting. The objective in the current study was to determine the most discriminating and predictive markers of autism spectrum disorders used in the Social Attention and Communication Study at 12, 18 and 24 months of age, so that these could be used to identify children with autism spectrum disorders with greater accuracy. The percentage of ‘yes/no’ responses for each behavioural marker was compared between children with autistic disorder (n = 39), autism spectrum disorder (n = 50) and developmental and/or language delay (n = 20) from 12 to 24 months, with a logistic regression also conducted at 24 months. Across all ages, the recurring key markers of both autistic disorder and autism spectrum disorder were deficits in eye contact and pointing, and from 18 months, deficits in showing became an important marker. In combination, these behaviours, along with pretend play, were found to be the best group of predictors for a best estimate diagnostic classification of autistic disorder/autism spectrum disorder at 24 months. It is argued that the identified markers should be monitored repeatedly during the second year of life by community health-care professionals.
