Dans la littérature existante, le « comportement externalisé » (p. ex., agressivité, opposition) d’un enfant autiste est souvent associé à de mauvais résultats en matière de santé mentale parentale. Pourtant, peu d’études ont examiné qualitativement l’ampleur et la nature de l’impact des comportements préjudiciables d’un enfant (préjudices physiques envers soi-même, autrui et les biens) sur les parents. Ici, nous avons examiné comment des parents autistes et non autistes conceptualisent les comportements préjudiciables de leurs enfants autistes, ainsi que l’impact de ces comportements sur les parents. Nous avons analysé 39 entretiens avec des parents autistes et non autistes au moyen d’une analyse thématique réflexive, à partir de laquelle nous avons identifié quatre thèmes. Le thème 1 englobait les expériences des parents concernant les comportements préjudiciables de leurs enfants. Le thème 2 rendait compte des explications des parents concernant les comportements préjudiciables de leurs enfants, qu’ils comprenaient comme indiquant une détresse intense. Dans le thème 3, les parents ont réfléchi à leurs réponses aux périodes de crise, au cours desquelles ils accordaient la priorité à la sécurité, mais reconnaissaient qu’un manque systémique de soutien entravait leurs efforts. Enfin, le thème 4 décrit les effets néfastes sur la santé mentale des parents, notamment des sentiments d’impuissance, de chagrin, de culpabilité et de peur pour l’avenir. Nous montrons qu’il est impératif de discuter plus ouvertement des comportements préjudiciables des enfants de manière neuroaffirmative, afin de développer des soutiens parentaux et familiaux réactifs et appropriés, qui reconnaissent le rôle de la culpabilité et du traumatisme dans la santé mentale parentale. Résumé grand public Beaucoup de recherches suggèrent que le comportement dit « externalisé » d’un enfant autiste (comme l’agressivité ou l’opposition) a un impact négatif sur la santé mentale des parents de cet enfant. Mais, jusqu’à présent, très peu d’études se sont entretenues directement avec des parents sur les raisons pour lesquelles et la manière dont ils éprouvent de la détresse en raison des comportements externalisés de leur enfant. Dans cette étude, nous avons interrogé 39 parents autistes et non autistes d’enfants autistes, afin de mieux comprendre leurs expériences de comportements externalisés spécifiques. Pour cette étude, nous nous sommes particulièrement intéressés aux « comportements préjudiciables » des enfants, que nous avons définis comme la violence envers soi-même, autrui et/ou les biens. Nous voulions que les parents nous décrivent avec leurs propres mots leurs expériences de ces comportements préjudiciables, la manière dont ils comprenaient ces périodes de crise, ce qu’ils faisaient pour réagir aux comportements préjudiciables de leurs enfants et ce qu’ils ressentaient à propos de ces comportements. Lorsque nous avons analysé les réponses des parents, nous avons dégagé quatre idées principales ou « thèmes ». Les parents considéraient les comportements préjudiciables comme une combinaison d’automutilation de l’enfant, de comportements suicidaires et de violence physique envers leurs parents, leurs frères et sœurs, et les biens (thème 1). Toutefois, les parents ne blâmaient pas nécessairement leur enfant, comprenant plutôt que le comportement de leur enfant était probablement une indication de détresse intense ou extrême, ou de dérégulation (thème 2). Les parents voulaient assurer la sécurité de toute leur famille, mais ils ont constaté qu’ils disposaient d’options limitées pour y parvenir, et ils estimaient qu’il y avait peu ou pas de soutien pour les aider durant les crises (thème 3). Malheureusement, les parents ont exprimé de nombreuses émotions différentes, telles que l’impuissance, le chagrin, la culpabilité et la peur, en conséquence des comportements préjudiciables de leur enfant (thème 4). Nous devons mieux comprendre ce qui se passe dans les familles, comment elles réagissent et de quel soutien elles ont besoin pour faire face, afin de pouvoir aider les familles qui vivent ce type de crises. Cet article aborde ces questions.
