Résumé
Lorsque le handicap est défini par le comportement, les chercheurs et les cliniciens peinent à identifier des mesures appropriées pour évaluer les progrès cliniques. Certains choisissent la réduction ou l'élimination des traits diagnostiques, définissant implicitement une apparence typique comme l'objectif de la prestation de services. Une telle approche interfère souvent avec des objectifs plus significatifs, centrés sur la personne ; nuit aux personnes handicapées ; et n'est pas nécessaire pour traiter des traits intrinsèquement nocifs ou personnellement pénibles, tels que l'automutilation. Les parties prenantes du handicap devraient réévaluer les mesures des résultats qui cherchent à éliminer des traits liés au handicap qui sont stigmatisés mais non nocifs. En utilisant l'autisme et le mouvement émergent de la neurodiversité comme étude de cas, cet article explore les défis éthiques liés à la sélection des mesures des résultats dans les diagnostics de handicap définis par le comportement.
When disability is defined by behavior, researchers and clinicians struggle to identify appropriate measures to assess clinical progress. Some choose the reduction or elimination of diagnostic traits, implicitly defining typical appearance as the goal of service provision. Such an approach often interferes with more meaningful, person-centered goals; causes harm to people with disabilities; and is unnecessary for dealing with traits that are intrinsically harmful or personally distressing, such as self-injury. Disability stakeholders should reevaluate outcome measures that seek to eliminate disability-related traits that are stigmatized but not harmful. Using autism and the emergent neurodiversity movement as a case study, this article explores ethical challenges in selecting outcome measures in behaviorally defined disability diagnoses.
