Résumé

RESUME A l'image de la population generale, le nombre d'etudiants en medecine, de medecins et, en effet, de psychiatres declarant etre neurodivergents augmente. Ces personnes possedent couramment diverses forces susceptibles d'ameliorer leur travail, mais ces forces peuvent ne pas etre reconnues. Trop souvent, ces personnes ont ete qualifiees de « medecins en difficulte ». Nous commencons cet article par une revue de la pensee contemporaine concernant la neurodiversite, avant d'examiner les problemes specifiques auxquels sont confrontes les medecins neurodivergents, en particulier les psychiatres. Nous explorons les forces neurodivergentes ainsi que les donnees probantes relatives aux resultats de carriere et a la sante mentale. Nous examinons la stigmatisation a laquelle de nombreux psychiatres neurodivergents sont confrontes dans la sphere medicale et la maniere dont les difficultes peuvent etre recadrees comme des besoins non satisfaits. Nous mettons en lumiere des initiatives visant a transformer la culture du lieu de travail, avant d'aborder le concept d'amenagements raisonnables, ainsi qu'un large eventail de suggestions pratiques d'amenagements a envisager, en utilisant le cadre Autistic SPACE et les orientations du Royal College of Psychiatrists sur les amenagements raisonnables. Enfin, nous examinons comment les personnes occupant des roles de direction superieure peuvent contribuer a ce domaine, servir de modeles et orienter vers des informations et un soutien supplementaires les medecins neurodivergents ainsi que leurs superviseurs et responsables hierarchiques directs.

SUMMARY Mirroring the general population, the number of medical students, doctors and, indeed, psychiatrists disclosing being neurodivergent is rising. These individuals commonly have a variety of strengths that can enhance their work, but these strengths may go unrecognised. All too often such individuals have been labelled ‘doctors in difficulty’. We begin this article with a review of contemporary thinking regarding neurodiversity, before considering specific issues facing neurodivergent doctors, specifically psychiatrists. We explore neurodivergent strengths and the evidence regarding career outcomes and mental health. We discuss the stigma that many neurodivergent psychiatrists face in the medical sphere and how difficulties may be reframed as unmet needs. We highlight initiatives that aim to change workplace culture, before discussing the concept of reasonable adjustments, alongside a wide range of practical suggestions of adjustments to consider, using the Autistic SPACE framework and the Royal College of Psychiatrists’ reasonable adjustments guidance. Finally, we consider how those in senior leadership roles can contribute to this field and provide role modelling and signposting to further information and support for neurodivergent doctors and their supervisors and line managers.
