Résumé

Objectif : L’objectif de l’étude était d’examiner les diagnostics de santé mentale et physique chez des adultes autistes sexuellement divers et/ou de genre divers (SGD). Méthodes : Nous avons mené une étude transversale à l’aide de données de dossiers de santé électroniques (DSE) de Kaiser Permanente Northern California. Les participants étaient des personnes ayant un diagnostic d’autisme inscrites au régime de santé entre 2015 et 2019. Les personnes SGD ont été identifiées à partir de données sur l’orientation sexuelle et l’identité de genre ou d’une méthodologie par étapes impliquant des recherches informatisées dans les DSE. Les codes diagnostiques relatifs aux affections de santé mentale et physique ont été examinés durant la période d’étude. Résultats : Parmi les 4159 participants autistes, n = 122 ont été identifiés comme sexuellement divers et n = 90 comme de genre divers. À l’aide de modèles de régression logistique multivariée ajustés selon l’âge, la race/l’origine ethnique et l’identité de genre, nous avons constaté que les groupes sexuellement divers et de genre divers présentaient tous deux des probabilités plus élevées de diagnostics de santé mentale que les personnes non identifiées comme SGD. Les adultes sexuellement divers présentaient des probabilités plus élevées de dépendance à l’alcool ou aux drogues, et les adultes de genre divers présentaient des probabilités plus élevées d’autres psychoses que les personnes non identifiées comme SGD. Les groupes sexuellement divers et de genre divers présentaient tous deux des probabilités plus élevées de certains diagnostics de santé physique, notamment de douleurs non classées ailleurs, de migraines et de troubles gastro-intestinaux, que les personnes non identifiées comme SGD. Conclusion : Les adultes autistes SGD présentaient des probabilités plus élevées de recevoir un diagnostic de certaines affections de santé mentale et physique que les adultes autistes non identifiés comme SGD, ce qui justifie des recherches supplémentaires sur les facteurs structurels et interpersonnels qui influent sur leur bien-être et sur la meilleure manière de les soutenir.

Purpose: The purpose of the study was to investigate mental and physical health diagnoses for sexually diverse and/or gender-diverse (SGD) autistic adults. Methods: We conducted a cross-sectional study using electronic health record (EHR) data from Kaiser Permanente Northern California. Participants were individuals with an autism diagnosis enrolled in the health plan between 2015 and 2019. SGD individuals were identified via sexual orientation and gender identity data or a stepwise methodology involving computerized searches of the EHR. Diagnostic codes for mental and physical health conditions were examined during the study period. Results: Of 4159 autistic participants, n = 122 were identified as sexually diverse and n = 90 as gender diverse. Using multivariable logistic regression models adjusted for age, race/ethnicity, and gender identity, we found that both the sexually diverse and gender diverse groups had greater odds of mental health diagnoses compared with individuals not identified as SGD. Sexually diverse adults had greater odds of alcohol or drug dependence, and gender-diverse adults had greater odds of other psychoses compared with individuals not identified as SGD. Both sexually diverse and gender diverse groups had greater odds of some physical health diagnoses, including pain not elsewhere classified, migraines, and gastrointestinal disorders, compared with individuals not identified as SGD. Conclusion: SGD autistic adults had greater odds of diagnosis of some mental and physical health conditions than autistic adults not identified as SGD, warranting further research on structural and interpersonal factors that affect their well-being and how best to support them.
