Résumé

Contexte : Malgré les bienfaits connus de l'activité physique, tels qu'un meilleur sommeil, un accroissement des liens sociaux et une diminution de la détresse psychologique, les adultes autistes n'atteignent souvent pas les niveaux recommandés. Les affections physiques et psychologiques coexistant fréquemment avec l'autisme, l'augmentation de l'activité physique représente une voie potentielle pour améliorer les résultats en matière de santé et de bien-être. Jusqu'à présent, la recherche sur l'activité physique s'est principalement concentrée sur les enfants et les adolescents autistes. L'objectif de cette étude était de comprendre la mise en oeuvre, la portée et la sécurité des programmes d'activité physique destinés aux adultes autistes. Méthodes : Nous avons mené une revue de portée. Présentée conformément aux lignes directrices PRISMA-ScR, notre revue incluait des études mettant en oeuvre des interventions d'activité physique auprès de personnes autistes âgées de 18 ans ou plus. Nous avons interrogé six bases de données jusqu'en juin 2024 à l'aide de mots-clés, de descripteurs MeSH et de dérivés relatifs à l'autisme et à l'activité physique. Nous avons extrait les données à l'aide d'un modèle standardisé, en appliquant une approche analytique descriptive afin de répartir les données dans des catégories associées à l'objectif de la revue. Résultats : Neuf études portant sur 148 adultes autistes (108 hommes ; âgés de 19 à 34 ans) répondaient aux critères d'inclusion. Les interventions d'activité physique mises en oeuvre comprenaient l'exercice aérobie, la marche ou le jogging, l'entraînement en force, l'entraînement des habiletés motrices, le football et l'exergaming. Des instructeurs formés, des professionnels de l'exercice ou de la santé, des étudiants, des entraîneurs, des assistants d'enseignement ou du personnel résidentiel ont supervisé des programmes menés à domicile, dans des écoles, des universités ou en milieu communautaire. Les stratégies utilisées pour soutenir la mise en oeuvre consistaient à intégrer des adaptations et un étayage appropriés, à utiliser des stratégies de soutien social et à favoriser l'autodétermination. Les éléments relatifs à la portée étaient décrits de manière variable parmi les études incluses. Les raisons de la portée réduite et de la non-participation comprenaient le manque d'intérêt, les contraintes d'emploi du temps ainsi que des problèmes médicaux et personnels. Les stratégies de sécurité comprenaient la réalisation d'un dépistage médical avant l'exercice et la fourniture d'une formation physique et psychologique aux personnes de soutien. Conclusion : La mise en oeuvre sûre de l'activité physique chez les adultes autistes est possible avec du soutien. Toutefois, les recherches portant sur l'activité physique chez les femmes autistes et les personnes ayant des besoins complexes sont insuffisantes.

Background: Despite the known benefits of physical activity—such as better sleep, increased social connection, and reduced psychological distress—autistic adults often do not meet recommended levels. As physical and psychological conditions often co-occur with autism, increasing physical activity represents a potential avenue for improved health and well-being outcomes. Physical activity research to date has primarily focused on autistic children and adolescents. The aim of this study was to understand the implementation, reach, and safety of physical activity programs for autistic adults. Methods: We conducted a scoping review. Reported according to PRISMA-ScR guidelines, our review included studies implementing physical activity interventions in autistic people aged ≥18 years. We searched six databases to June 2024 with keywords, MeSH headings and derivatives relating to autism and physical activity. We extracted data using a standardized template, applying a descriptive analytical approach to map data to categories associated with the review’s aim. Results: Nine studies involving 148 autistic adults (108 male; aged 19–34 years) met inclusion criteria. Physical activity interventions implemented included aerobic exercise, walking or jogging, strength training, motor skills training, soccer, and exer-gaming. Trained instructors, exercise or health professionals, students, coaches, teaching assistants, or residential staff supervised programs conducted in home, school, university, or community settings. Strategies used to support implementation were incorporating appropriate adaptations and scaffolding, using social support strategies and facilitating self-determination. Elements of reach were described variably among the included studies. Reasons for reduced reach and nonparticipation included lack of interest, scheduling, and medical and personal issues. Safety strategies included conducting pre-exercise medical screening and providing physical and psychological training for support people. Conclusion: Safe implementation of physical activity among autistic adults is possible with support. However, there is a lack of research addressing physical activity among autistic women and those with complex needs.
