Nous avons cherché à quantifier le risque, la mortalité et la charge du suicide chez les personnes autistes. Nous avons effectué une recherche dans PubMed, Embase et PsycINFO le 5 avril 2023 afin d'identifier des sources rapportant le risque relatif (RR) de suicide ou de tentative de suicide chez les personnes autistes (enregistrement PROSPERO : CRD42021265313). La prévalence du spectre de l'autisme ainsi que la mortalité par suicide et les années de vie perdues (YLL) provenaient de l'étude Global Burden of Disease 2021. Les RR regroupés par méta-régression et les estimations des indicateurs de santé ont été utilisés pour estimer l'excès de mortalité par suicide et les YLL chez les personnes autistes. Nous avons identifié 983 études uniques, dont dix répondaient aux critères d'inclusion, comprenant 10,4 millions de personnes. Le RR regroupé du suicide chez les personnes autistes était de 2,85 (UI à 95 % : 2,05-4,03), et était significativement plus élevé chez les femmes autistes que chez les hommes autistes. Aucune preuve de biais de publication n'a été détectée par l'inspection du graphique en entonnoir et le test d'Egger. À l'échelle mondiale, nous avons estimé à 13 400 le nombre de décès par suicide excédentaires chez les personnes autistes en 2021, ce qui équivaut à 1,8 % de l'ensemble des décès par suicide et à 621 000 YLL excédentaires. Les études étaient limitées en nombre et en couverture géographique. Des stratégies efficaces de prévention du suicide chez les personnes autistes pourraient réduire considérablement la charge mortelle des suicides à l'échelle mondiale et réduire la charge sanitaire subie par cette population.
