Résumé

CONTEXTE : Les adolescents atteints de troubles genetiques rares representent une population pouvant etre exposee a un risque sur le plan psychosocial. Malgre son importance, l'association entre les traits autistiques et le retrait social dans les problemes comportementaux et emotionnels chez les adolescents atteints de troubles genetiques rares reste peu etudiee. OBJECTIF : L'etude visait a examiner empiriquement les caracteristiques cliniques d'adolescents atteints de troubles genetiques rares selon une approche guidee par la theorie du comportement. METHODE : Nous avons etudie les problemes comportementaux et emotionnels ainsi que les traits autistiques actuels et au cours de la vie dans un echantillon de 93 adolescents norvegiens (Mage = 13,2 ans, SDage = 2,4, tranche d'age 10-17 ans, 62,4 % de filles, 37,6 % de garcons) atteints de divers troubles genetiques rares. Les adolescents ont ete etudies de maniere transversale au moyen de questionnaires psychometriques standardises evalues par leurs parents. RESULTATS : Davantage de traits autistiques actuels et au cours de la vie, ainsi que le retrait social, etaient tous associes a davantage de problemes internalises. Des analyses supplementaires ont montre que le retrait social mediait partiellement l'association positive entre les traits autistiques actuels et les problemes internalises. En revanche, le retrait social mediait entierement l'association positive entre les traits autistiques au cours de la vie et les problemes internalises. CONCLUSION ET IMPLICATIONS : Nos resultats mettent en evidence des caracteristiques importantes des adolescents atteints de troubles genetiques rares qui peuvent orienter les cliniciens et les interventions futures. Le retrait social peut etre prodromique de problemes internalises tels que l'anxiete et la depression. Ainsi, la prise en charge clinique du retrait social peut constituer un moyen de prevenir les problemes internalises chez les adolescents atteints de troubles genetiques rares et presentant des traits autistiques.

BACKGROUND: Adolescents with rare genetic disorders represent a population that can be at risk in psychosocial terms. Despite its importance, the association of autism traits and social withdrawal in behavioral and emotional problems in adolescents with rare genetic disorders remains understudied. AIM: The study aimed to empirically examine the clinical characteristics of adolescents with rare genetic disorders with a behavior theory-driven approach. METHOD: We investigated the behavioral and emotional problems and current and lifetime autistic traits in a sample of 93 Norwegian adolescents (Mage = 13.2 years, SDage = 2.4, rangeage 10-17, 62.4% females, 37.6% males) with various rare genetic disorders. The adolescents were investigated cross-sectionally utilizing standardized psychometric questionnaires rated by their parents. RESULTS: More current and lifetime autistic traits and social withdrawal were all associated with more internalizing problems. Further analyses demonstrated that social withdrawal partially mediated the positive association between current autistic traits and internalizing problems. In contrast, social withdrawal fully mediated the positive association between lifetime autistic traits and internalizing problems. CONCLUSION AND IMPLICATIONS: Our results demonstrate important characteristics of adolescents with rare genetic disorders that may guide clinicians and future interventions. Social withdrawal may be prodromal to internalizing problems such as anxiety and depression. Thus, clinically addressing social withdrawal can represent a means to prevent internalizing problems in adolescents with rare genetic disorders and autistic traits.
