Résumé

Importance : L’intervention auprès des personnes présentant un trouble du spectre de l’autisme (TSA) commence généralement après le diagnostic. À ce jour, aucun essai d’une intervention administrée à des nourrissons avant le diagnostic n’a montré d’effet sur les résultats diagnostiques. Objectif : Déterminer l’efficacité d’une intervention préventive pour le TSA débutant durant la période prodromique. Conception, cadre et participants : Cet essai clinique randomisé, mené dans 2 sites et avec un évaluateur en aveugle, d’une intervention préventive comparée aux soins habituels, a été réalisé dans 2 centres de recherche australiens (Perth, Melbourne). Un échantillonnage communautaire a été utilisé pour recruter 104 nourrissons âgés de 9 à 14 mois présentant des comportements précoces associés ultérieurement au TSA, mesurés à l’aide du Social Attention and Communication Surveillance-Revised. Le recrutement s’est déroulé du 9 juin 2016 au 30 mars 2018. Les données du suivi final ont été recueillies le 15 avril 2020. Interventions : Les nourrissons ont été randomisés selon un ratio de 1:1 afin de recevoir soit une intervention préventive associée aux soins habituels, soit les soins habituels seuls, sur une période de 5 mois. Le groupe d’intervention préventive a reçu une intervention de communication sociale en 10 séances, iBASIS-Video Interaction to Promote Positive Parenting (iBASIS-VIPP). Les soins habituels comprenaient des services fournis par des cliniciens communautaires. Principaux critères de jugement et mesures : Les nourrissons ont été évalués au départ (âge approximatif, 12 mois), à la fin du traitement (âge approximatif, 18 mois), à l’âge de 2 ans et à l’âge de 3 ans. Le critère de jugement principal était la mesure combinée, en aveugle, de la sévérité des comportements liés au TSA (l’Autism Observation Scale for Infants et l’Autism Diagnostic Observation Schedule, deuxième édition) aux 4 temps d’évaluation. Les critères secondaires étaient un diagnostic clinique indépendant de TSA, en aveugle, à l’âge de 3 ans, ainsi que des mesures du développement de l’enfant. Les analyses ont été préenregistrées et comprenaient des tests unilatéraux avec un niveau alpha de 0,05. Résultats : Parmi les 171 nourrissons évalués pour l’éligibilité, 104 ont été randomisés ; 50 nourrissons (âge chronologique moyen [ET], 12,40 [1,93] mois ; 38 garçons [76,0 %]) ont reçu l’intervention préventive iBASIS-VIPP associée aux soins habituels (1 nourrisson a été exclu après la randomisation), et 53 nourrissons (âge moyen [ET], 12,38 [2,02] mois ; 32 garçons [60,4 %]) ont reçu les soins habituels seuls. Au total, 89 participants (45 dans le groupe iBASIS-VIPP et 44 dans le groupe de soins habituels) ont été réévalués à l’âge de 3 ans. L’intervention iBASIS-VIPP a entraîné une réduction de la sévérité des symptômes du TSA (aire entre les courbes, -5,53 ; IC à 95 %, moins l’infini à -0,28 ; P = 0,04). Une réduction des probabilités de classification TSA à l’âge de 3 ans a été observée dans le groupe iBASIS-VIPP (3 participants sur 45 [6,7 %]) par rapport au groupe de soins habituels (9 participants sur 44 [20,5 %] ; rapport de cotes, 0,18 ; IC à 95 %, 0 à 0,68 ; P = 0,02). Le nombre de participants à traiter pour réduire la classification TSA était de 7,2. Des améliorations de la réactivité des aidants et des résultats langagiers ont également été observées dans le groupe iBASIS-VIPP. Conclusions et pertinence : La réception d’une intervention préventive pour le TSA dès l’âge de 9 mois chez un échantillon de nourrissons présentant des signes précoces de TSA a entraîné une réduction de la sévérité des symptômes du TSA pendant la petite enfance et a réduit les probabilités d’un diagnostic de TSA à l’âge de 3 ans. Importance: Intervention for individuals with autism spectrum disorder (ASD) typically commences after diagnosis. No trial of an intervention administered to infants before diagnosis has shown an effect on diagnostic outcomes to date. Objective: To determine the efficacy of a preemptive intervention for ASD beginning during the prodromal period. Design, Setting, and Participants: This 2-site, single rater-blinded randomized clinical trial of a preemptive intervention vs usual care was conducted at 2 Australian research centers (Perth, Melbourne). Community sampling was used to recruit 104 infants aged 9 to 14 months showing early behaviors associated with later ASD, as measured by the Social Attention and Communication Surveillance-Revised. Recruitment occurred from June 9, 2016, to March 30, 2018. Final follow-up data were collected on April 15, 2020. Interventions: Infants were randomized on a 1:1 ratio to receive either a preemptive intervention plus usual care or usual care only over a 5-month period. The preemptive intervention group received a 10-session social communication intervention, iBASIS-Video Interaction to Promote Positive Parenting (iBASIS-VIPP). Usual care comprised services delivered by community clinicians. Main Outcomes and Measures: Infants were assessed at baseline (approximate age, 12 months), treatment end point (approximate age, 18 months), age 2 years, and age 3 years. Primary outcome was the combined blinded measure of ASD behavior severity (the Autism Observation Scale for Infants and the Autism Diagnostic Observation Schedule, second edition) across the 4 assessment points. Secondary outcomes were an independent blinded clinical ASD diagnosis at age 3 years and measures of child development. Analyses were preregistered and comprised 1-tailed tests with an α level of.05. Results: Of 171 infants assessed for eligibility, 104 were randomized; 50 infants (mean [SD] chronological age, 12.40 [1.93] months; 38 boys [76.0%]) received the iBASIS-VIPP preemptive intervention plus usual care (1 infant was excluded after randomization), and 53 infants (mean [SD] age, 12.38 [2.02] months; 32 boys [60.4%]) received usual care only. A total of 89 participants (45 in the iBASIS-VIPP group and 44 in the usual care group) were reassessed at age 3 years. The iBASIS-VIPP intervention led to a reduction in ASD symptom severity (area between curves, -5.53; 95% CI, -∞ to -0.28; P =.04). Reduced odds of ASD classification at age 3 years was found in the iBASIS-VIPP group (3 of 45 participants [6.7%]) vs the usual care group (9 of 44 participants [20.5%]; odds ratio, 0.18; 95% CI, 0-0.68; P =.02). Number needed to treat to reduce ASD classification was 7.2 participants. Improvements in caregiver responsiveness and language outcomes were also observed in the iBASIS-VIPP group. Conclusions and Relevance: Receipt of a preemptive intervention for ASD from age 9 months among a sample of infants showing early signs of ASD led to reduced ASD symptom severity across early childhood and reduced the odds of an ASD diagnosis at age 3 years.
