Résumé

CONTEXTE : Il est rapporté que les personnes sur le spectre de l'autisme présentent des altérations de l'interoception, le sens de l'état interne du corps. Le Questionnaire sensoriel de l'interoception (ISQ) est une mesure d'auto-évaluation de l'interoception comportant 20 items, spécifiquement destinée à mesurer ce construit chez les personnes autistes. Les propriétés psychométriques de l'ISQ n'avaient toutefois pas auparavant été évaluées dans un large échantillon de personnes autistes. MÉTHODES : À l'aide d'une analyse factorielle confirmatoire, nous avons évalué la structure latente de l'ISQ dans un large échantillon en ligne d'adultes sur le spectre de l'autisme et avons constaté que le modèle unidimensionnel s'ajustait mal aux données. À l'aide d'une analyse de mauvaise spécification pour identifier les zones de mauvais ajustement local et de la théorie de réponse à l'item pour étudier l'adéquation de l'échelle de réponse à sept points, nous avons supprimé les items redondants et regroupé les options de réponse afin de proposer un nouvel ISQ à huit items et à cinq choix de réponse. RÉSULTATS : L'ISQ révisé à cinq choix de réponse (ISQ-8) a montré un ajustement bien meilleur tout en maintenant une fiabilité interne élevée. Les analyses de fonctionnement différentiel des items (DIF) ont indiqué que les items de l'ISQ-8 recevaient des réponses comparables de la part des adolescents et des adultes autistes, ainsi qu'au sein de plusieurs autres groupes sociodémographiques. LIMITES : Nos résultats étaient limités par le fait que nous n'avons pas recueilli de données auprès de témoins au développement typique, ce qui empêchait l'analyse du DIF selon le statut diagnostique. De plus, bien que cette étude propose une nouvelle échelle à 5 réponses pour l'ISQ-8, nos données n'ont pas été recueillies à l'aide de cette méthode ; ainsi, les propriétés psychométriques de la version révisée de cet instrument nécessitent des investigations supplémentaires. CONCLUSION : L'ISQ-8 est prometteur en tant que mesure fiable et valide de l'interoception chez les adolescents et les adultes sur le spectre de l'autisme, mais des travaux supplémentaires sont nécessaires pour examiner ses propriétés psychométriques dans cette population. BACKGROUND: Individuals on the autism spectrum are reported to display alterations in interoception, the sense of the internal state of the body. The Interoception Sensory Questionnaire (ISQ) is a 20-item self-report measure of interoception specifically intended to measure this construct in autistic people. The psychometrics of the ISQ, however, have not previously been evaluated in a large sample of autistic individuals. METHODS: Using confirmatory factor analysis, we evaluated the latent structure of the ISQ in a large online sample of adults on the autism spectrum and found that the unidimensional model fit the data poorly. Using misspecification analysis to identify areas of local misfit and item response theory to investigate the appropriateness of the seven-point response scale, we removed redundant items and collapsed the response options to put forth a novel eight-item, five-response choice ISQ. RESULTS: The revised, five-response choice ISQ (ISQ-8) showed much improved fit while maintaining high internal reliability. Differential item functioning (DIF) analyses indicated that the items of the ISQ-8 were answered in comparable ways by autistic adolescents and adults and across multiple other sociodemographic groups. LIMITATIONS: Our results were limited by the fact that we did not collect data for typically developing controls, preventing the analysis of DIF by diagnostic status. Additionally, while this study proposes a new 5-response scale for the ISQ-8, our data were not collected using this method; thus, the psychometric properties for the revised version of this instrument require further investigation. CONCLUSION: The ISQ-8 shows promise as a reliable and valid measure of interoception in adolescents and adults on the autism spectrum, but additional work is needed to examine its psychometrics in this population. A free online score calculator has been created to facilitate the use of ISQ-8 latent trait scores for further studies of autistic adolescents and adults (available at https://asdmeasures.shinyapps.io/ISQ_score/ ).
