Résumé

Bien que l'auto-efficacité parentale et le phénotype autistique élargi (BAP) aient été associés à la dépression, à l'anxiété et au stress des aidants à des moments précis, leur influence sur les trajectoires de santé mentale à plus long terme demeure inconnue, en particulier chez les aidants qui participent à des programmes de soutien pour leurs enfants présentant des traits autistiques très précoces. Grâce à une analyse secondaire des données d'un essai contrôlé randomisé longitudinal, utilisant une analyse de courbe de croissance latente, nous avons exploré le schéma et les prédicteurs personnels des trajectoires de dépression, d'anxiété et de stress des aidants sur une période de deux ans dans un échantillon de 100 familles d'enfants présentant des traits autistiques précoces (âgés de 12 mois à 3 ans), dont les familles avaient été randomisées pour recevoir un programme de soutien médiatisé par les parents ou les soins communautaires habituels. Alors que les aidants présentaient en moyenne des niveaux stables et faibles de dépression, d'anxiété et de stress, chez certaines personnes, des niveaux stables et élevés de ces symptômes étaient associés à une faible satisfaction parentale ainsi qu'à des traits BAP plus marqués de difficultés de langage pragmatique et de rigidité. Aucun élément n'indiquait que la participation au programme de soutien médiatisé par les aidants avait prédit les résultats de santé mentale des aidants. Les aidants d'enfants présentant des traits autistiques précoces qui présentent eux-mêmes des symptômes de santé mentale précoces et persistants pourraient bénéficier d'un soutien supplémentaire spécifique en santé mentale. • Des traits BAP plus marqués chez les aidants étaient associés à une augmentation des symptômes de santé mentale. • Une satisfaction parentale plus élevée était associée à une diminution des symptômes de santé mentale. • Un programme de soutien médiatisé par les aidants n'a pas modifié la santé mentale des aidants. • Les aidants présentant des symptômes de santé mentale pourraient avoir besoin d'un soutien supplémentaire.

While parenting self-efficacy and broader autism phenotype (BAP) have been linked to caregiver depression, anxiety and stress at specific points in time, their influence on longer-term mental health trajectories remains unknown, especially for caregivers who participate in support programs for their infants with very-early autistic features. Through secondary analysis of a longitudinal randomized control trial dataset, using latent growth curve analysis we explored the pattern and personal predictors of caregiver depression, anxiety and stress trajectories over a two-year period in a sample of 100 families of infants with early autistic features (when aged 12 months to 3 years) where families had been randomized to receive a parent-mediated support program or community care as usual. While caregivers were estimated to have stable, low-level depression, anxiety and stress on average, for some individuals, stable and high levels of these were associated with low parenting satisfaction, and higher BAP traits of pragmatic language difficulties and rigidity. There was no evidence that participation in the caregiver-mediated support program had predicted caregiver mental health outcomes. Caregivers of infants with early autism features who themselves present with early, sustained mental health symptoms may benefit from additional specific mental health support. • Higher caregiver BAP traits associated with increased mental health symptoms. • Higher parenting satisfaction associated with decreased mental health symptoms. • A caregiver-mediated support program did not change caregiver mental health. • Caregivers with mental health symptoms may need additional support.
