Résumé Objectif En Aotearoa Nouvelle-Zélande (NZ), les familles font face à des obstacles persistants pour accéder à un soutien précoce fondé sur des données probantes pour les enfants autistes. Cet essai contrôlé randomisé (ECR) a évalué l’efficacité de Let’s Play ; un programme de soutien précoce sur mesure, médiatisé par les aidants, destiné aux enfants autistes et à leurs aidants. Méthodes Cet ECR en simple aveugle (évaluateur) incluait 91 dyades parent-enfant, réparties aléatoirement dans le programme Let’s Play (soutien actif ; AS [n = 45]) ou dans un groupe témoin sur liste d’attente (WLC ; n = 46). Les participants étaient des aidants d’enfants âgés de 0 à 5 ans ayant un diagnostic formel ou des caractéristiques d’autisme. Let’s Play a été dispensé pendant 9 semaines au moyen d’ateliers de groupe et d’un accompagnement à domicile. Les principaux résultats chez l’enfant et l’aidant, évalués au départ, après le soutien et au suivi à 6 mois, comprenaient respectivement l’engagement parent-enfant et le stress parental. Résultats Les enfants ont montré des indications descriptives d’amélioration de l’engagement aidant-enfant, de la qualité de vie liée à la santé et du comportement entre le départ et l’après-soutien. Une amélioration du stress parental, des symptômes de dépression et d’anxiété, ainsi que de la compétence parentale auto-perçue, était également évidente aux différents temps d’évaluation. Toutefois, aucun effet significatif Groupe x Temps n’a été observé pour l’engagement aidant-enfant, le nombre d’énoncés ou le nombre de mots différents. Un effet Groupe x Temps était apparent pour toutes les autres variables de résultat chez l’enfant et l’aidant, soulignant les bénéfices de Let’s Play. Conclusions Cette recherche fournit des données préliminaires sur l’efficacité d’un programme de soutien précoce de faible intensité, communautaire et médiatisé par les aidants, pour la qualité de vie liée à la santé et le comportement des enfants autistes ainsi que le bien-être des aidants. Toutefois, des approches plus ciblées ou soutenues pour favoriser l’engagement aidant-enfant et la communication vocale pourraient être nécessaires pour qu’une amélioration soit observée. Enregistrement de l’essai clinique Le protocole de recherche a été enregistré de manière prospective dans le Registre australien et néo-zélandais des essais cliniques (ACTRN12622001139763).
