Résumé

Dans le trouble du spectre de l’autisme (TSA), les affections médicales durant la petite enfance pourraient constituer des marqueurs prédictifs d’un diagnostic ultérieur de TSA. Dans cette étude, les dossiers médicaux électroniques de 579 personnes autistes et de 1897 témoins appariés avant l’âge de 2 ans ont été analysés afin d’identifier des affections potentiellement prédictives. À l’aide d’un nouvel outil, l’association relative de chaque affection dans le groupe autiste a été comparée à celle du groupe témoin au moyen de régressions logistiques appliquées aux dossiers médicaux. L’épilepsie convulsive généralisée, le nystagmus, l’absence de développement physiologique normal, le retard des étapes du développement et le strabisme étaient plus probables chez les personnes ayant reçu ultérieurement un diagnostic de TSA, tandis que l’ictère périnatal était moins susceptible d’être associé. Des affections moins connues, telles que le strabisme et le nystagmus, pourraient indiquer de nouveaux profils prédictifs d’affections concomitantes susceptibles d’améliorer les pratiques de dépistage du TSA.

In autism spectrum disorder (ASD), medical conditions in infancy could be predictive markers for later ASD diagnosis. In this study, electronic medical records of 579 autistic individuals and 1897 matched controls prior to age 2 were analyzed for potential predictive conditions. Using a novel tool, the relative association of each condition in the autistic group was compared to the control group using logistic regressions across medical records. Generalized convulsive epilepsy, nystagmus, lack of normal physiological development, delayed milestones, and strabismus were more likely in those later diagnosed with ASD while perinatal jaundice was less likely to be associated. Lesser-known conditions, such as strabismus and nystagmus, may point to novel predictive co-occurring condition profiles which could improve screening practices for ASD.
