Résumé

L'intersection fréquente entre l'autisme et les identités transgenres a été décrite dans des contextes cliniques et communautaires. Cette étude examine les neurophénotypes liés à l'autisme chez les jeunes transgenres. Quarante-cinq jeunes transgenres, répartis de manière égale entre des sous-groupes non autistes, légèrement autistes au seuil subclinique et autistes répondant à l'ensemble des critères, ont réalisé une imagerie par résonance magnétique fonctionnelle à l'état de repos afin d'examiner la connectivité fonctionnelle. Les résultats ont confirmé l'hyperconnectivité hypothétisée des nœuds du réseau du mode par défaut (DMN) avec les réseaux visuel et moteur dans l'autisme, reproduisant partiellement des études antérieures comparant des adolescents autistes et non autistes cisgenres. Le groupe légèrement autiste au seuil subclinique différait à la fois des groupes non autiste et autiste répondant à l'ensemble des critères quant à la connectivité des nœuds du DMN avec les réseaux attentionnel ventral et sensorimoteur, se situant entre les groupes non autiste et autiste répondant à l'ensemble des critères. Les traits autistiques ont montré un schéma similaire aux analyses de groupes liées à l'autisme et étaient également liés à une hyperconnectivité entre le nœud du DMN et le réseau attentionnel dorsal. L'intériorisation, la dysphorie de genre et la stigmatisation liée au statut de minorité de genre n'ont pas montré de différences de connectivité. Les différences de connectivité au sein du DMN suivaient des schémas précédemment rapportés selon le sexe assigné à la naissance (c'est-à-dire que l'assignation féminine à la naissance montrait une connectivité plus importante au sein du DMN). Dans l'ensemble, les résultats suggèrent que les diagnostics comportementaux et les traits autistiques chez les jeunes transgenres correspondent à des différences observables de connectivité des nœuds du DMN. En outre, cette étude révèle de nouvelles caractéristiques neurophénotypiques associées à l'autisme légèrement sous le seuil, soulignant l'importance d'une attention de recherche portée à ce groupe.

The common intersection of autism and transgender identities has been described in clinical and community contexts. This study investigates autism-related neurophenotypes among transgender youth. Forty-five transgender youth, evenly balanced across non-autistic, slightly subclinically autistic, and full-criteria autistic subgroupings, completed resting-state functional magnetic resonance imaging to examine functional connectivity. Results confirmed hypothesized default mode network (DMN) hub hyperconnectivity with visual and motor networks in autism, partially replicating previous studies comparing cisgender autistic and non-autistic adolescents. The slightly subclinically autistic group differed from both non-autistic and full-criteria autistic groups in DMN hub connectivity to ventral attention and sensorimotor networks, falling between non-autistic and full-criteria autistic groups. Autism traits showed a similar pattern to autism-related group analytics, and also related to hyperconnectivity between DMN hub and dorsal attention network. Internalizing, gender dysphoria, and gender minority-related stigma did not show connectivity differences. Connectivity differences within DMN followed previously reported patterns by designated sex at birth (i.e. female birth designation showing greater within-DMN connectivity). Overall, findings suggest behavioral diagnostics and autism traits in transgender youth correspond to observable differences in DMN hub connectivity. Further, this study reveals novel neurophenotypic characteristics associated with slightly subthreshold autism, highlighting the importance of research attention to this group.
