Résumé

Les troubles du sommeil affectent 50 % a 80 % des personnes autistes et sont associes a un risque accru de comorbidites psychiatriques et de comportements dangereux, notamment l automutilation, les idees suicidaires et l agressivite. Bien que des etudes precedentes aient identifie des relations correlationnelles, peu ont examine si les problemes de sommeil predisent les comportements dangereux dans de grands echantillons issus du monde reel. Cette etude a utilise des donnees cliniques retrospectives provenant d une organisation a but non lucratif de sante comportementale afin d examiner si les problemes de sommeil predisent les comportements dangereux chez les personnes autistes, en tenant compte de l age et du fonctionnement adaptatif. L echantillon comprenait 8 375 personnes autistes (age moyen = 8,35 ans). Les problemes de sommeil, les comportements dangereux et le fonctionnement adaptatif ont ete mesures lors de l admission et tous les 6 mois. Des regressions logistiques binaires ont evalue si les problemes de sommeil predisaient les idees suicidaires actuelles et passees, l automutilation et l agressivite. Les resultats ont montre que les problemes de sommeil augmentaient significativement les chances d avoir des antecedents d idees suicidaires et des idees suicidaires lors de l admission (rapport de cotes = 2,29 et 2,46, respectivement), des antecedents d automutilation et une automutilation lors de l admission (rapport de cotes = 1,95 et 2,20, respectivement), ainsi que de l agressivite lors de l admission (rapport de cotes = 1,58), meme apres ajustement pour l age, le sexe et le fonctionnement adaptatif. L age et le comportement adaptatif etaient egalement associes independamment aux comportements dangereux. Ces resultats soulignent l importance d evaluer et de traiter regulierement les problemes de sommeil chez les personnes autistes, en tant que voie potentielle pour reduire les comportements dangereux et ameliorer le bien etre global. Resume destine au grand public Les problemes de sommeil sont tres frequents chez les enfants et les adultes autistes, touchant entre la moitie et les trois quarts des personnes. Un mauvais sommeil est lie a des risques plus eleves de comportements graves tels que l automutilation, les pensees suicidaires et l agressivite, mais il est mal compris si les problemes de sommeil predisent reellement ces comportements. Cette etude a examine des donnees cliniques issues du monde reel provenant de plus de 8 000 personnes autistes, agees de 2 a 21 ans, qui ont recu des services d une grande organisation de sante comportementale. Nous avons examine si le fait d avoir des problemes de sommeil augmentait la probabilite de comportements dangereux, meme apres prise en compte de l age de la personne, de ses competences de vie quotidienne et de son sexe. Nous avons constate que les personnes ayant des problemes de sommeil etaient plus de deux fois plus susceptibles de presenter une automutilation ou des pensees suicidaires et etaient egalement plus susceptibles de presenter de l agressivite. Ces resultats mettent en evidence l importance pour les prestataires et les familles d evaluer regulierement les difficultes de sommeil et d y remedier dans le cadre de la prise en charge de l autisme. L amelioration du sommeil peut constituer une etape importante vers la reduction du risque de comportements dangereux et le soutien du bien etre global des personnes autistes et de leurs familles.

Sleep disturbances affect 50%–80% of autistic individuals and are linked to heightened risk for psychiatric comorbidities and dangerous behaviors, including self-injury, suicidal ideation, and aggression. While previous studies have identified correlational relationships, few have examined whether sleep problems predict dangerous behaviors in large, real-world samples. This study used retrospective clinical data from a behavioral health nonprofit to investigate whether sleep problems predict dangerous behaviors among autistic individuals, accounting for age and adaptive functioning. The sample included 8,375 autistic individuals (mean age = 8.35 years). Sleep problems, dangerous behaviors, and adaptive functioning were measured at intake and every 6 months. Binary logistic regressions assessed whether sleep problems predicted current and historical suicidal ideation, self-injury, and aggression. Results showed that sleep problems significantly increased the odds of a history of suicidal ideation and suicidal ideation at intake (odds ratio = 2.29, 2.46, respectively), a history of self-injury and self-injury at intake (odds ratio = 1.95, 2.20, respectively), and aggression at intake (odds ratio = 1.58), even after adjusting for age, sex, and adaptive functioning. Age and adaptive behavior were also independently associated with dangerous behaviors. These findings underscore the importance of routinely assessing and treating sleep problems in autistic individuals as a potential pathway to reducing dangerous behaviors and improving overall wellbeing. Lay Abstract Sleep problems are very common among autistic children and adults, affecting between half and three-quarters of individuals. Poor sleep is linked to higher risks for serious behaviors such as self-injury, suicidal thoughts, and aggression, but it is not well understood whether sleep problems actually predict these behaviors. This study looked at real-world clinical data from over 8,000 autistic individuals, ages 2 to 21, who received services from a large behavioral health organization. We examined whether having sleep problems increased the likelihood of dangerous behaviors, even after taking into account a person’s age, daily living skills, and sex. We found that individuals with sleep problems were more than twice as likely to show self-injury or suicidal thoughts and were also more likely to show aggression. These findings highlight how important it is for providers and families to regularly assess and address sleep difficulties as part of autism care. Improving sleep may be an important step toward reducing the risk of dangerous behaviors and supporting overall wellbeing for autistic individuals and their families.
