Résumé

Résumé Objectif L'analyse appliquée du comportement est l'intervention la plus courante pour les enfants autistes. Il est proposé depuis longtemps qu'un plus grand nombre d'heures par semaine d'ABA produise de meilleurs résultats. Les lignes directrices de pratique de l'ABA recommandent généralement 30 à 40 h par semaine pour les jeunes enfants autistes. Bien que certaines études étayent un effet de dosage, d'autres études ne constatent aucune relation prédictive entre les heures de traitement ABA et un éventail de résultats chez l'enfant. Ainsi, la question du dosage reste ouverte, laissant les parents et les prestataires incertains quant au dosage idéal d'ABA permettant à un enfant autiste de réaliser des progrès significatifs. Les données du monde réel (RWD) peuvent combler l'écart entre les traitements communautaires et la recherche contrôlée souvent menée par les universités. Méthodes Dans cette étude, nous avons examiné les données RWD de 725 enfants autistes recevant une ABA dans le cadre de soins communautaires. À l'aide d'une modélisation longitudinale multiniveau, nous avons examiné la relation entre le nombre moyen d'heures d'ABA par mois et l'évolution des mesures proximales et distales : comportement adaptatif, atteinte des objectifs et comportements dangereux. Résultats Le dosage était un prédicteur significatif de l'atteinte des objectifs, ce qui indique que les clients qui passaient davantage de temps en traitement atteignaient davantage d'objectifs. À l'inverse, la relation était négative pour le comportement adaptatif et la communication, de sorte qu'un nombre d'heures plus élevé prédisait des progrès plus lents dans ces domaines. Enfin, les comportements dangereux diminuaient au fil du temps et étaient indépendants du nombre d'heures. Des analyses secondaires ont montré que les capacités de communication au départ prédisaient les résultats du traitement au-delà du dosage, sauf pour les comportements dangereux. Conclusion Bien que davantage de recherches soient nécessaires, cette étude suggère que les lignes directrices de pratique devraient modérer le langage concernant le dosage et l'ABA.

Abstract Purpose Applied behavioral analysis is the most common intervention for autistic children. It has long been proposed that more hours per week in ABA produce better outcomes. ABA practice guidelines typically recommend 30–40 h per week for young autistic children. While some studies support a dosage effect, other studies find no predictive relationship between ABA treatment hours and a range of child outcomes. Thus, the dosage question remains open, leaving parents and providers unsure of the ideal dosage of ABA for an autistic child to make meaningful progress. Real world data (RWD) can fill the gap between community treatment and controlled research often conducted by universities. Methods In this study, we examined the RWD data from 725 autistic children receiving ABA as part of community-based care. Using longitudinal multilevel modeling, we examined the relationship between average hours of ABA per month and change on proximal and distal measures: adaptive behavior, goal attainment, and dangerous behavior. Results Dosage was a significant predictor of goal attainment, indicating that clients who spent more time in treatment met more goals. Conversely, the relationship was negative for adaptive behavior and communication, such that higher hours predicted slower progress in these domains. Finally, dangerous behavior decreased over time and was independent of hours. Secondary analyses demonstrated that communication abilities at baseline predicted treatment outcomes above and beyond dosage, except for dangerous behavior. Conclusion While more research is needed, this study suggests that practice guidelines should temper language around dosage and ABA.
