Résumé

Résumé La qualité des soins dans les écoles publiques et d'autres contextes communautaires destinés aux jeunes d'âge scolaire sur le spectre de l'autisme est variable et souvent non fondée sur des données probantes. Former les praticiens de ces contextes à dispenser des pratiques fondées sur des données probantes (EBP) peut améliorer la qualité des soins. Nous avons développé un système gratuit de formation et d'orientation clinique sur Internet synthétisant plusieurs EBP pour les jeunes sur le spectre de l'autisme, ciblant divers besoins en santé mentale et comportements liés à l'autisme, intitulé Modular EBPs for Youth on the Autism Spectrum (MEYA ; meya.ucla.edu). Une étude à lignes de base multiples a été menée auprès de sept praticiens recrutés dans des contextes de pratique en santé mentale à travers les États-Unis, qui fournissaient des services à des enfants sur le spectre de l'autisme (âgés de 6 à 17 ans). Les praticiens ont été assignés aléatoirement à des conditions de ligne de base de 2 à 8 semaines. Une fois la formation en ligne à MEYA commencée, les praticiens ont suivi une autoformation guidée par algorithme aux EBP pour l'autisme. Les participants ont enregistré les séances sur vidéo. Des évaluateurs indépendants ont utilisé l'échelle de fidélité MEYA (MEYA-FS) pour évaluer l'adhésion et la compétence relatives aux EBP pour l'autisme. Les praticiens ont également rempli des mesures relatives aux résultats de la mise en oeuvre et les parents ont évalué les résultats des jeunes concernant des comportements cibles personnalisés. Cinq praticiens sur sept ont accru leur adhésion aux pratiques MEYA (c'est-à-dire les scores MEYA-FS) après la formation MEYA. Les résultats relatifs à la compétence étaient similaires, bien que quelque peu moins solides. Les praticiens ont généralement considéré MEYA comme faisable, compréhensible et acceptable. La plupart des résultats chez les jeunes se sont améliorés pendant MEYA. Un essai randomisé contrôlé de MEYA serait utile pour caractériser son efficacité à soutenir la mise en oeuvre des EBP par les praticiens et les résultats chez les jeunes dans les contextes de services scolaires et communautaires.

Abstract The quality of care in public schools and other community settings for school-aged youths on the autism spectrum is variable and often not evidence-based. Training practitioners in these settings to deliver evidence-based practices (EBPs) may improve the quality of care. We developed a free internet-based training and clinical guidance system synthesizing multiple EBPs for youth on the autism spectrum addressing a range of mental health needs and autism-related behaviors, entitled Modular EBPs for Youth on the Autism Spectrum (MEYA; meya.ucla.edu). A multiple baseline study was conducted with seven practitioners recruited from mental health practice settings across the United States who were providing services to children on the autism spectrum (aged 6 to 17 years). Practitioners were randomly assigned to undergo baseline conditions of 2 to 8 weeks. Once online training in MEYA commenced, practitioners engaged in algorithm-guided self-instruction in EBPs for autism. Participants video-recorded sessions. Independent coders used the MEYA Fidelity Scale (MEYA-FS) to rate adherence and competence in EBPs for autism. Practitioners also completed measures pertaining to implementation outcomes and parents rated youth outcomes on personalized target behaviors. Five of seven practitioners increased their adherence to MEYA practices (i.e., MEYA-FS scores) following MEYA training. Findings for competence were similar, though somewhat less robust. Practitioners generally viewed MEYA as feasible, understandable, and acceptable. Most youth outcomes improved during MEYA. A randomized, controlled trial of MEYA would be helpful in characterizing its effectiveness for supporting practitioner EBP implementation and youth outcomes in school and community service settings.
