Résumé

Le trouble du spectre de l'autisme (TSA) et le trouble déficitaire de l'attention avec hyperactivité (TDAH) sont les troubles neurodéveloppementaux les plus fréquemment diagnostiqués. Bien que la comorbidité ait été exclue dans le DSM-IV (APA, 2000), le DSM-5 (APA, 2013) n'empêche pas le diagnostic concomitant de TSA et de TDAH (TSA+TDAH). Cette étude visait à comprendre les distinctions entre les déficits exécutifs associés à ces affections. Nous avons utilisé l'analyse du temps de réaction (TR) et des potentiels liés aux événements (PLE) pendant la réalisation d'une tâche d'oddball comportant des figures illusoires. Les participants étaient des enfants (N = 18 par groupe) présentant un TSA, un TDAH, un TSA+TDAH, ainsi que des témoins neurotypiques (CNT). L'analyse a révélé que les groupes TSA et TSA+TDAH commettaient davantage d'erreurs et présentaient des taux plus élevés d'erreurs par omission. Le TR après erreur dans les groupes TSA et TSA+TDAH se manifestait par une accélération de la réponse après erreur plutôt que par le ralentissement normatif du TR. Les groupes TSA et TSA+TDAH ont montré une négativité liée à l'erreur (ERN) atténuée par rapport au groupe TDAH et aux témoins. La N100 frontale était augmentée en réponse aux figures cibles et non cibles dans les groupes TSA et TSA+TDAH. Les PLE frontaux présentaient des latences prolongées dans le groupe TDAH par rapport aux autres groupes. L'étude a confirmé l'utilité des PLE pour élucider les différences entre le TSA et le TDAH ainsi que leur impact dans le diagnostic double. Ces informations contribuent à définir l'étendue du chevauchement entre ces affections, tant en termes d'expression des symptômes que de neuropathologie sous-jacente.

Autism spectrum disorder (ASD) and attention-deficit/hyperactivity disorder (ADHD) are the most commonly diagnosed neurodevelopmental disorders. Although the comorbidity was excluded in DSM-IV (APA, 2000), DSM‑5 (APA, 2013) does not preclude the concurrent diagnosis of ASD and ADHD (ASD+ADHD). This study aimed to understand distinctions in executive deficits among these conditions. We used analysis of reaction time (RT) and event-related potentials (ERP) during performance on oddball task with illusory figures. Participants were children (N = 18 per group) with ASD, ADHD, ASD+ADHD, and neurotypical controls (CNT). Analysis revealed that ASD and ASD+ADHD groups committed more errors and had higher omission error rates. Post‑error RT in ASD and ASD+ADHD manifested as a post-error response speeding rather than normative RT slowing. The ASD and ASD+ADHD demonstrated an attenuated error-related negativity (ERN) as compared to ADHD and controls. The frontal N100 was enhanced to both target and nontarget figures in ASD and ASD+ADHD groups. Frontal ERPs had prolonged latencies in the ADHD as compared to other groups. The study confirmed the utility of using ERP to elucidate differences between ASD and ADHD and their impact in dual diagnosis. This information helps define the extent of overlap among these conditions both in terms of symptom expression and underlying neuropathology.
