Résumé

La cooccurrence de l'autisme et de la diversité de genre a été de plus en plus étudiée au cours de la dernière décennie. On estime qu'environ 11 % des personnes transgenres et de genre divers (TGD) reçoivent un diagnostic d'autisme. Cependant, les connaissances concernant des soins cliniques appropriés liés au genre pour les personnes TGD autistes sont insuffisantes. Nous avons réalisé une revue exploratoire des orientations cliniques actuelles relatives aux soins des personnes TGD afin d'identifier ce qui était dit au sujet de l'autisme. Les documents d'orientation clinique ont été recherchés dans PubMed, Web of Science, Google Scholar, Embase, Guidelines International Network et la base de données médicale TRIP, ainsi que par l'exploration des références et la recommandation d'experts. Les données probantes ont été synthétisées par synthèse narrative, cartographie des recommandations et analyse de la fréquence des références. Parmi les 31 documents d'orientation clinique identifiés, seuls onze mentionnaient spécifiquement l'intersection entre l'autisme et les personnes TGD. Les concepts clés parmi les recommandations disponibles comprenaient la promotion d'une approche multidisciplinaire ; la mise en évidence de l'intersectionnalité de l'autisme et des expériences de diversité de genre lors des évaluations ; et, surtout, la reconnaissance du fait que l'autisme, en soi, ne constitue pas un critère d'exclusion pour recevoir des soins liés au genre. Toutefois, des orientations cliniques détaillées et pratiques font défaut en raison d'une lacune dans les données probantes. Des recherches empiriques sur les expériences et les résultats en matière de soins des personnes TGD autistes, utilisant une approche développementale, couvrant l'ensemble de la vie et fondée sur les forces, sont nécessaires pour produire des orientations adaptées et éclairées par les données probantes. The co-occurrence of autism and gender diversity has been increasingly studied in the past decade. It is estimated that ∼11% of transgender and gender-diverse (TGD) individuals are diagnosed with autism. However, there is insufficient knowledge about appropriate gender-related clinical care for autistic TGD individuals. We performed a scoping review of current clinical guidance for the care of TGD individuals to identify what was said about autism. Clinical guidance documents were searched in PubMed, Web of Science, Google Scholar, Embase, Guidelines International Network, and TRIP medical database, as well as reference mining and expert recommendation. Evidence was synthesised by narrative synthesis, recommendation mapping, and reference frequency analysis. Out of the identified 31 clinical guidance documents, only eleven specifically mentioned the intersection between autism and TGD. Key concepts among the available recommendations included advocating for a multidisciplinary approach; emphasising the intersectionality of autism and gender-diverse experiences during assessments; and-importantly-recognising that autism, in itself, does not serve as an exclusion criterion for receiving gender-related care. However, detailed and practical clinical guidance is lacking due to a gap in evidence. Empirical research into the care experiences and outcomes of autistic TGD individuals using a developmental, lifespan, and strengths-based approach is needed to generate evidence-informed and tailored guidance.
