Résumé

Nous examinons le rapport coût-efficacité du traitement de l’épilepsie par des médicaments antiépileptiques chez les enfants autistes, en considérant les répercussions sur les prestataires de soins de santé (en Angleterre, en Irlande, en Italie et en Espagne) et sur les familles des enfants (en Irlande). Nous constatons que la carbamazépine est le médicament le plus coût-efficace à essayer en premier chez les enfants présentant des crises focales nouvellement diagnostiquées. Pour l’Angleterre et l’Espagne, l’oxcarbazépine est le traitement le plus coût-efficace lorsqu’elle est prise comme traitement additionnel pour les enfants dont la réponse à la monothérapie est sous-optimale. En Irlande et en Italie, la gabapentine est l’option la plus coût-efficace. Notre analyse de scénarios supplémentaire présente le coût global pour les familles ayant des enfants autistes qui sont traités pour l’épilepsie : ce coût est considérablement plus élevé que les dépenses des prestataires de soins de santé.

We examine the cost-effectiveness of treating epilepsy with anti-epileptic medicines in autistic children, looking at impacts on healthcare providers (in England, Ireland, Italy and Spain) and children's families (in Ireland). We find carbamazepine to be the most cost-effective drug to try first in children with newly diagnosed focal seizures. For England and Spain, oxcarbazepine is the most cost-effective treatment when taken as additional treatment for those children whose response to monotherapy is suboptimal. In Ireland and Italy, gabapentin is the most cost-effective option. Our additional scenario analysis presents the aggregate cost to families with autistic children who are being treated for epilepsy: this cost is considerably higher than healthcare provider expenditure.
